摘　　要：目的:探讨医疗服务能力视角下"河南省省直医疗卫生机构医疗服务能力提升工程"项目经费使用效率,为提高临床重点专科专项资金使用效率提供依据。方法:根据临床重点专科建设定位和《三级综合医院医疗服务能力指南》,确定项目经费使用效率评价指标。运用数据包络分析和聚类分析进行经费使用效率评价。结果:河南省16个国家临床重点专科综合技术效率、纯技术效率、规模效率平均值分别为0.923、0.975、0.945,其中8个专科综合技术效率为1.000。31个省级临床重点专科综合技术效率、纯技术效率、规模效率平均值分别为0.911、0.960、0.947,其中13个专科综合技术效率为1.000。两组专科综合技术效率、纯技术效率、规模效率、规模报酬差异无统计学意义。结论:临床重点专科经费使用效率总体情况较好,仍有专科需改善提高。应合理配置经费规模及使用结构,提升特色医疗技术水平。Objective:To explore the fund usage efficiency of the project of"Medical Service Ability Improvement Project of Henan Provincial Direct Medical and Health Institutions"from the perspective of medical service ability,and to provide a basis for improving the efficiency of special funds for key clinical specialties.Methods:According to the construction orientation of key clinical specialties and the Guidelines for Medical Service Capability of Tertiary General Hospitals,the evaluation indicators for the efficiency of project funds use were determined.It used data envelopment analysis and cluster analysis to evaluate the use efficiency of funds.Results:The average values of comprehensive technical efficiency,pure technical efficiency,and scale efficiency averages of16 national key clinical specialties were 0.923,0.975,and 0.945,respectively,of which the comprehensive technical efficiency of 8 specialties was 1.000.The average values of comprehensive technical efficiency,pure technical efficiency,and scale efficiency of the31 provincial key clinical specialties were 0.911,0.960,and 0.947,respectively,of which the comprehensive technical efficiency of13 specialties was 1.000.There was no statistically significant difference in comprehensive technical efficiency,pure technical efficiency,scale efficiency,and return to scale between these two specialties.Conclusion:The overall efficiency of the use of funds for key clinical specialties was good.There were still subjects that need to be improved.It was necessary to rationally allocate the scale of funds,use structure,and to improve the level of characteristic medical technology.
