摘　　要：目的探究电针丰隆穴对高脂血症患者的降脂疗效及对巨噬细胞与人血清共培养Janus激酶2/信号传导和转录激活子3信号通路/锌指蛋白36(JAK2/STAT3/TTP信号通路)的影响。方法将90例辨证为痰浊阻遏证高脂血症患者,随机分为电针组、药物组与对照组,每组30例。电针组予以电针针刺丰隆穴治疗;药物组予以口服血脂康治疗;对照组予以非经非穴针刺治疗。观察治疗前后及治疗后1月、2月、6月3组患者总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL-C)、低密度脂蛋白(LDL-C)的水平变化;取各组患者血清,与人THP-1巨噬细胞共培养,并采用实时定量聚合酶链反应法(RT-PCR)及免疫印迹法(Western Blotting)检测巨噬细胞JAK2、STAT3、TTP mRNA和蛋白表达,并加以比较。结果治疗结束后,电针组总有效率83.3%,药物组总有效率86.6%,两者改善血脂总有效率无统计学差异(P>0.05);电针组与药物组TC、TG、LDL-C下降水平比较,差异无统计学意义(P>0.05)。与治疗后比较,治疗后2月、治疗后6月,药物组TC、TG、LDL-C有所升高,差异有统计学意义(P<0.05),电针组未见明显差异(P>0.05)。治疗后电针组与药物组巨噬细胞JAK2、STAT3、TTP及蛋白表达较对照组均显著升高(P<0.05);电针组与药物组比较,两者变化无明显差异(P>0.05)。结论电针丰隆穴可降低患者TC、TG、LDL-C水平,且疗效较药物持久,可促进巨噬细胞JAK2、STAT3、TTP mRNA和蛋白的表达,促进胆固醇逆转运,减弱炎症活动,由此实现对动脉粥样硬化性心脑血管疾病的有效防治。Objective To observe the clinical effect of electro-acupuncture at ST40 on hyperlipidemia,and the Janus kinase 2/signal transducer and activator of transcription 3 signaling pathway/Tristetraprolin(JAK2/STAT3/TTP)in macrophages.Methods A total of 90 patients with hyperlipidemia were randomly divided into electro-acupuncture group,Xuezhikang group and control group,with 30 patients in each group.Electroacupuncture at ST40 was manipulated in the electroacupuncture group patients;the Xuezhikang tablets were administered for patients in the Xuezhikang group patients;electroacupuncture at the non-acupoint was undergone in the control group.The levels of triglyceride(TG),total cholesterol(TC),high density lipoprotein cholesterol(HDL-C)and low-density lipoprotein cholesterol(LDL-C)were detected before and after treatment,and 1,2 and 6 months after the treatment.Serum of each group was cocultured with human THP-1 macrophages.The mRNAs and proteins of JAK2,STAT3,TTP in macrophages were detected by RT-PCR and Western blot.Results After treatment,the total effective rate of electroacupuncture group was83.3%,and 86.6%in Xuezhikang group.There was no statistical difference in the total effective rate of improving blood lipid between the two groups(P>0.05).There was no statistical difference in the decrease of TG,TC and LDL-C between electroacupuncture group and Xuezhikang group(P>0.05).Compared to the end of the treatment,TC,TG and LDL in Xuezhikang group increased significantly 2 months after the treatment,while there was no significant change in electroacupuncture group.The levels of JAK2,STAT3,TTP mRNA and protein in macrophages in electro-acupuncture group and Xuezhikang group were significantly higher than those in control group(P<0.05),and there was no significant difference between EA group and Xuezhikang group(P>0.05)at the end of the treatment.Conclusion Electroacupuncture at ST40 can reduce the levels of TG,TC and LDL-C,and its effect can maintain a longer than Xuezhikang.It may promote the expression of JAK2,STAT3,
