摘　　要：目的探讨采用分期手术方式治疗婴儿PattersonⅢA型先天性束带综合征的临床效果。方法对2014年1月至2018年12月中国人民解放军空军军医大学第一附属医院骨科收治的4例PattersonⅢA型先天性束带综合征患儿的临床资料进行回顾性分析。其中男2例,女2例,初次就诊年龄7~19 d。采用分期手术治疗一期行局部麻醉下指端分离术,手术时患儿平均年龄14.5(7~19)d;二期行全身麻醉下分指术后残端畸形整形术,同时切除四肢其余环状缩窄带,手术时患儿平均年龄10.2(8~12)个月。二期手术后平均随访21.0(15~30)个月。采用Moses等提出的标准对患手功能与患肢外形进行评估。结果4例患儿两次手术切口均一期愈合。末次随访时患指抓握等功能与患肢外形良好,优4例,优良率100%。结论对PattersonⅢA型先天性束带综合征患儿,行分期手术治疗可获得较好的患手功能和患肢外观,效果令人满意。Objective To investigate the clinical effect of staged surgical treatment for infant with PattersonⅢA congenital constriction band syndrome.Methods Four infants with congenital constriction band syndrome admitted to the Department of Orthopaedics,the First Affiliated Hospital of PLA Air Force Military Medical University and treated by staged surgery from January 2014 to December 2018 were retrospectively collected.There were 2 males and 2 females,aged 7-19 d.In the neonatal period,local anesthesia and division of the fingertips were performed.In the second stage,the fingertips were reshaped and the remaining deformities were removed under general anesthesia at average age of 10.2(8-12)months.The postoperative follow-up duration was 15-30 months with an average of 21.0 months.The function of affected fingers and appearance of postoperative limbs were evaluated using the criteria proposed by Moses,et al.Results All the surgical incisions of the 4 patients were healed smoothly.At the last follow-up,the appearance of the affected limbs and the function of the affected fingers were excellent in 4 cases,and the excellent and good rate was 100%.Parents of the 4 children were satisfied.Conclusion The staged surgical treatment of PattersonⅢA congenital constriction band syndrome in infants can achieve satisfactory function and appearance of the affected fingers and limbs.
