摘 要:目的探讨不同大小甲状腺髓样癌(medullary thyroid carcinoma, MTC)的超声特征。方法回顾性分析术后病理证实的52个MTC结节的超声、临床资料,以同期笔者医院术后病理证实的104个甲状腺乳头状癌(papillary thyroid carcinoma, PTC)结节作为对照组,并根据结节大小将MTC及PTC分为>10mm及≤10mm组,分析各组MTC与PTC超声特征异同。结果与PTC比较,MTC结节较大(1.92cm vs 1.05cm,P<0.05),MTC边缘规则、血流信号丰富、纵横比<1更多见(P均<0.05)。结节最大径≤10mm组,与PTC比较,MTC纵横比<1更多见(P<0.05)。结节最大径>10mm组,与PTC比较,MTC等回声、边缘规则、血流信号丰富、纵横比<1更多见(P均<0.05)。结论 MTC与PTC超声特征有相似之处,如低回声、实性、有钙化以微钙化为主,但MTC较PTC边缘规则、血流信号丰富、纵横比<1更多见。Objective To investigate the ultrasonographic features of different sizes of medullary thyroid carcinoma(MTC).Methods A retrospective analysis was conducted on the 52 MTC nodules pathologically confirmed after operation for both ultrasonographic data and clinical data,with 104 nodules of papillary thyroid carcinoma(PTC)pathologically confirmed after operation in our hospital during the same period as the control group.For both MTC and PTC groups,the nodules were divided into>10mm group and≤10mm group according their sizes,so as to analyze the similarities and differences between MTC and PTC ultrasonographic features of the groups.Results MTC nodules were larger than PTC nodules(1.92cm vs 1.05cm,P<0.05).Compared with PTC,MTC had regular margin,rich vascularity and more common aspect ratio<1(all P<0.05).For the≤10mm(the maximum diameter of nodule)group,the aspect ratio<1 of MTC was more common than that of PTC(P<0.05).For the>10mm(the maximum diameter of nodule)group,compared with PTC,MTC had isoechoic,regular margin,rich vascularity and more common aspect ratio<1(all P<0.05).Conclusion MTC and PTC had similar ultrasonographic features,such as hypoechoic,solid,calcified(dominated by micro-calcification).However,compared with PTC,MTC had regular margin,rich vascularity and more common aspect ratio<1.
