摘 要:目的:探讨CT半自动分割技术评价的腹部脂肪分布特征与泌尿系结石患病的相关性。方法:运用CT半自动分割技术测量泌尿系结石患者和无泌尿系结石患者的腰围(WL)、腹部最大横径(ATD)、腹部最大矢状面前后径(ASD)、皮下脂肪厚度(SAT)、腹腔内脏脂肪面积(VAA)、腹壁皮下脂肪面积(SAA)、内脏脂肪面积百分比(VAA/(VAA+SAA));比较相关指标在两组人群中的差异,分析不同指标与结石患病的相关性,评价不同指标对结石患病风险的预测效能。结果:病例组与对照组年龄、性别构成比差异无统计学意义(P>0.05),两组人群BMI、ASD、VAA、SAA、VAA/(VAA+SAA)指标差异有统计学意义(P<0.05),WL、ATD、SAT指标差异无统计学意义(P>0.05);相关性分析显示BMI、ATD、ASD、SAA与结石患病呈低度相关,VAA、VAA/(VAA+SAA)与结石患病呈中度相关,WL、SAT与结石患病无明显相关;ROC分析显示VAA和VAA/(VAA+SAA)预测结石患病风险的AUC大于其他指标(分别为0.830和0.867)。结论:CT半自动分割技术可有效评价腹部脂肪分布特征,其中内脏脂肪分布与泌尿系结石发生有相关性。Objective:To analyze the correlation between abdominal adipose distribution and urinary tract stones using CT semi-automatic segmentation technology.Methods:Using CT semi-automatic segmentation technology to measure the waistline(WL),abdominal maximum transverse diameter(ATD),abdominal maximum sagittal diameter(ASD),subcutaneous adipose thickness(SAT),visceral adipose area(VAA),subcutaneous adipose area(SAA),visceral adipose area percentage(VAA/(VAA+SAA));compare the differences of the indicators between the two groups and analyze the correlation between different indicators and urinary tract stones.Results:There was no significant difference in age and gender between the stone group and normal group(P>0.05).The differences in the indicators of BMI,ASD,VAA,SAA and VAA/(VAA+SAA)between the two groups were statistically significant(P<0.05),while the differences in the indicators of WL,ATD and SAT were not statistically significant(P>0.05).Correlation analysis showed that the indicators of BMI,ATD,ASD,SAA had lowly correlation with urinary tract stones,while the indicators of VAA and VAA/(VAA+SAA)had moderately correlation with urinary tract stones.ROC analysis showed that the AUC of VAA and VAA/(VAA+SAA)to predict the risk of stone disease were greater than other indicators(0.830 and0.867 respectively).Conclusion:CT semi-automatic segmentation technology can effectively evaluate the characteristics of abdominal fat distribution.Visceral adipose distribution is related to the occurrence of urinary tract stones.
