基于古今医案分析桃核承气汤治疗小便异常的证治规律

摘　　要：目的归纳与探讨桃核承气汤治疗小便异常的证治规律。方法手工检阅文献中运用桃核承气汤治疗小便不利的相关医案,采用Excel 2016软件建立数据库并导入古今医案云平台(V1.4),采用频次分析与关联分析,对所纳入的数据进行分析统计。结果本研究共纳入76例医案,桃核承气汤治疗的小便异常主要见于癃闭、血淋、尿血、石淋等病症,其病机主要为:瘀热互结,热重而血瘀轻,辨证以瘀热互结证为主,主要症状为:小便异常,大便秘结,小腹满痛;舌象以舌质红或黯红、舌苔黄为常见,脉象以脉弦数或沉涩为常见。常配伍凉血清热、活血化瘀、利尿通淋之品。结论桃核承气汤治疗小便异常的辨证要点为:小便异常,大便秘结,小腹满痛,舌质红或黯红、舌苔黄,脉弦数或沉涩。Objective To summarize and explore the rules of the syndrome and treatment of Taohe Chengqi Decoction in treating dysuria.Method Searching by hands to find any literature including medical records related to treatments of dysuria using Taohe Chengqi Decoction.Using Excel 2016 software to establish database and importing the ancient and modern medical case cloud platform(v1.4).Statistics analysis was made for the included data using frequency and correlation analysis.Result A total of 76 medical records were included in this study.The urinary abnormalities treated by Taohe Chengqi Decoction were mainly found in obliteration,hemorrhage,blood in urine,stone leaching and other diseases.The main pathogenesis of the disease was:stasis and heat,heat and blood.Light stasis,syndrome differentiation is mainly based on the mutual syndrome of stasis and heat.The main symptoms are:abnormal urination,constipation,abdominal pain;red or dark tongue tongue is common,and tongue coating is yellow.Pulse is often pulse string or heavy.Astringency is common.It is often compatible with products that cool serum heat,promote blood circulation and remove blood stasis,diuresis and relieve leaching.Conclusion The key points of syndrome differentiation of Taohe Chengqi Decoction in the treatment of dysuria are dysuria,constipation,abdominal pain,red or dark red tongue,yellow tongue coating,and wiry-rapid or deep-choppy pulse.
