摘 要:目的探讨肝癌缺失基因1(DLC-1)启动子区CpG岛甲基化在多发性骨髓瘤(MM)患者中的表达及相关临床意义。方法采用重亚硫酸盐测序PCR(BSP)法定量检测自2016年10月—2017年12月在新疆医科大学附属肿瘤医院初诊的30例MM患者DLC-1基因的甲基化状态,分别进行MM患者的临床资料分析、MM患者及健康对照组关于DLC-1基因甲基化情况的比较,以及MM患者DLC-1基因甲基化情况与相关临床指标的比较。结果(1)临床资料有关M蛋白的免疫分型提示:30例MM患者中IgG型21例(21/30),IgA型6例(6/30),轻链型3例(3/30);骨髓细胞学检查提示:MM患者中骨髓瘤细胞比例<30%的有6例(21/30),30%~50%之间的有19例(19/30),≥50%的有5例(5/30)。(2)MM组与对照组比较提示:MM组DLC-1启动子区甲基化率显著高于正常组(P=0.009),其中CpG岛4、6、7、13、21位点的甲基化率显著高于正常对照组(P值分别为0.017、0.030、0.023、0.017、0.044)。(3)进一步对MM患者深入比较发现,DLC-1基因的甲基化比例分别在Ⅱa+Ⅲa期与Ⅲb期组间、血清肌酐正常与升高患者组间、骨髓异常浆细胞/骨髓瘤细胞比例50%≥与<50%组间均存在统计学差异(P均<0.05)。结论DLC-1异常甲基化可能与肿瘤负荷及肾功能损害相关,深入研究有关DLC-1基因甲基化在MM患者中的表达情况,可为今后MM的去甲基化治疗提供新的研究思路。Objective To explore the expression and clinical significance of Deleted in Liver Cancer 1 gene(DLC-1)methylation in multiple myeloma(MM).Methods Themethylation status of DLC-1 gene was detected by Bisulfite sequencing PCR(BSP)in the MM patients since October 2016 to December 2017,from the hospital.Then the clinical data of them were analyzed,the expression of DLC-1 gene methylation of the two groups were compared,and the correlation between DLC-1 gene methylation and some clinical indicators of the MM patients were investigated.Results(1)In the 30 MM cases,there were 21 IgG type(21/30),6 IgA type(6/30)and 3 light chain type(3/30)respectively.Bone marrow examination indicated that there were 6(6/30),19(19/30)and 5(5/30)in MM patients with plasma cell proportion<30%,30%~50%,and≥50%in bone marrow respectively.(2)The methylation rate of DLC-1 gene in the MM group was significantly higher than that of the control group(P=0.009),and the methylation rates of CpG island 4,6,7,13 and 21 were significantly higher than that of the control group(P=0.017,0.030,0.023,0.017 and 0.044 respectively).(3)There were statistical differences in the proportion of DLC-1 methylation between stage Ⅱa+Ⅲa and Ⅲb,between patients with normal and elevated creatinine,and between the proportion of bone marrow myeloma cells in≥50%group and<50%group(all P<0.05).Conclusion Abnormal DLC-1 methylation may be related to tumor load and renal impairment.In-depth study on the expression of DLC-1 gene methylation in the MM patients can provide a new research idea for the demethylation treatment of it in the future.
