银翘温胆汤联合头孢曲松钠、万古霉素治疗急性重症肺炎的临床疗效及对炎性因子和血气分析指标的影响

摘　　要：目的探讨银翘温胆汤联合头孢曲松钠、万古霉素对急性重症肺炎患者临床疗效及转化生长因子-β(TGF-β)、降钙素原(PCT)和C-反应蛋白(CRP)水平、血气分析指标的影响。方法选取130例急性重症肺炎患者,随机分为观察组与对照组。对照组给予头孢曲松钠、万古霉素治疗,观察组同时加用中药银翘温胆汤治疗,两组均连续治疗10 d。统计两组治疗后临床疗效;检测并比较两组治疗前后炎性因子TGF-β、PCT、CRP水平及血气分析指标二氧化碳分压(PaCO2)、动脉血氧分压(PaO2)、动脉血氧饱和度(SaO2)、氧合指数(PaO2/FiO2)水平;统计不良反应发生情况。结果观察组治疗后总有效率高于对照组(P<0.05);治疗后两组血清TGF-β水平及PaO2、SaO2和PaO2/FiO2水平较本组治疗前升高,且观察组高于对照组(P<0.05);两组治疗后血清PCT、CRP水平及PaCO2水平均较本组治疗前降低,且观察组低于对照组(均P<0.05);与本组治疗前比较,两组治疗后慢性健康状况评分系统Ⅱ(APACHEⅡ)评分降低,且观察组低于对照组(P<0.05)。两组不良反应发生率比较差异无统计学意义(P>0.05)。结论银翘温胆汤联合头孢曲松钠、万古霉素可有效调控急性重症肺炎患者血清TGF-β、PCT因子水平,恢复PaO2、SaO2、PaCO2和PaO2/FiO2水平,进而控制病情进展,提升临床疗效,且应用安全性好。Objective To investigate the effects of Yinqiao Wendan Decoction combined with ceftriaxone sodium and vancomycin on the levels of transforming growth factor-β(TGF-β),procalcitonin(PCT)and C-reactive protein(CRP)in patients with acute severe pneumonia The influence of blood gas analysis indicators.Methods A total of 130 patients with acute severe pneumonia treated were selected as the research subjects.They were divided into observation group and control group according to the random number table method.The control group was treated with ceftriaxone sodium and vancomycin.The observation group was treated with Yinqiao Wendan decoction on the basis of the control group.Both groups were treated continuously for 10 d.Statistics of the clinical efficacy of the two groups after treatment;detection and comparison of the levels of inflammatory factors TGF-β,PCT,CRP and blood gas analysis indicators of carbon dioxide(PaCO2),arterial partial pressure of oxygen(PaO2),and arterial blood oxygen before and after treatment.Saturation(SaO2),oxygenation index(PaO2/FiO2)levels and adverse reactions were calculated.Results The total effective rate of the observation group after treatment was higher(P<0.05).After treatment,the serum TGF-βlevels and PaO2,SaO2 and PaO2/FiO2 levels of the two groups were higher than those before the treatment,and the observation group was higher than the control group(P<0.05).Serum PCT,CRP levels and PaCO2 levels after treatment in the two groups were lower than those before treatment in this group,and the observation group was lower than that in the control group(all P<0.05);Compared with this group before treatment,the two groups after treatment,the chronic health status scoring systemⅡ(APACHEⅡ)score decreased,and the observation group was lower than the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Yinqiao Wendan Decoction combined with ceftriaxone sodium and vancomycin can effectively regulate serum T
