摘 要:目的探讨新型冠状病毒肺炎(COVID-19)CT表现及定量CT肺功能对COVID-19肺功能评估。方法回顾性分析31例经病毒核酸检测确诊、且具有完整动态胸部HRCT资料的COVID-19患者,复查间隔时间48 h~14天。分析HRCT基本表现及CT-肺功能成像与临床分型的关系。结果31例患者均于发病3天内接受HRCT检查,病变检出率为100%。于发病5~7天复查CT,25例病变范围增大,3例病灶范围缩小,3例病灶范围同前;发病10~14天接受第2次复查,26例病变范围缩小,3例病灶范围增大;临床分型与CT肺功能值(PI)(r=-0.835,P<0.001)呈显著负相关。结论胸部HRCT能清晰显示COVID-19疾病影像学变化,CT肺功能与COVID-19相关,有助于指导临床早诊断、早治疗,并可评价COVID-19的临床治疗效果。Objective To explore the CT manifestations of(COVID-19)and to assess the diagnostic value of quantitative CT pulmonary function in the clinical classification of corona virus disease 2019.Methods Totally,31 COVID-19 patients confirmed by virus nucleic acid detection were collected.All patients had completed dynamic chest HRCT imaging data.The relation between the quantitative CT pulmonary function and clinical classification of corona virus disease 2019 were analyzed.Results All 31 patients underwent chest HRCT within 3 days after onset.The decectable rate of COVID-19 was 100%(31/31).About 5~7 days after onset,all the patients underwent the first time re-examination of HRCT,the lesion scope enlarged in 21 cases(21/31,67.74%)but reduced in 6 cases(6/31,19.35%),the lesion range remained the same like before in 4 cases(4/31,12.90%)and about 10~14 days after onset,all the patients underwent the second time re-examination of HRCT,the lesion scope reduced in 26 cases(26/31,83.87%)but enlarged in 3 cases(3/31,9.67%).There was a significant negative correlation between the quantitative CT pulmonary function and the clinical classification of COVID-19.Conclusion HRCT can clearly show imaging changes of the COVID-19 in time.The quantitative CT pulmonary function was correlated with the clinical classification of COVID-19,which is helpful to guiding early clinical diagnosis,early treatment,and providing evidence for evaluating the effect of clinical treatment of COVID-19.
