泌尿外科专科护理门诊建立及其在术前预康复中的实践

摘　　要：目的建立泌尿外科专科护理门诊及探讨其在术前预康复中的实践效果。方法建立泌尿外科独立护理门诊,选择符合资质的护士坐诊,建立术前预康复工作模式并实施,包括对患者进行运动疗法、营养支持和心理干预。由患者、医生、护士对泌尿外护理门诊进行评价。结果共选拔2名护士出诊,泌尿外科专科护理门诊开展以来共对163例患者实施术前康复干预,主要集中于泌尿系肿瘤、前列腺增生、泌尿结石疾病患者,其中对18例肺功能不全患者给予运动疗法指导,对65例患者实施营养支持干预,对10例存在焦虑患者症转介给心理医生。患者总体满意度为96.3%。医生认为,泌尿外护理门诊的开展能够加强术前患者营养支持,使患者建立充足的身体功能储备;开展术前预康复的运动疗法,将有助于提升患者心功能与肺功能的耐力。护士认为,专科护理门诊可为患者提供了多元化术前预康复护理服务、提升护士自我价值。结论专科护理门诊可为患者提供多元化术前预康复护理服务,使患者建立充足的身体功能储备,提升患者心功能与肺功能的耐力,对提高患者术后康复效果具有积极的意义,同时提升护士的自我价值感。Objective To establish the specialized nursing clinic in the department of urology and study its effect on preoperative prehabilitation.Methods A specialized nursing clinic was set up in the department of urology,and qualified nurses were selected to provide clinical nursing consultations for patients and meanwhile build a mode for preoperative prehabilitation and practice it,including kinesitherapy,nutritional support,and psychological intervention.The specialized outpatient nursing service will be evaluated by the patients,doctors,and nurses.Results Two qualified nurses were selected to do the nursing consultations.Totally,163 patients were intervened with preoperative rehabilitation,most of whom were those patients with urinary tumors,prostatic hyperplasia,and urinary calculi,since its establishment.Among them,18 patients with pulmonary incompetence received kinesitherapy,65 received nutritional support;and 10 with anxiety disorders were introduced to psychologists.The overall satisfaction among the patients was 96.3%.According to the doctors,the specialized nursing clinic in the department of urology improved the preoperative nutritional support,and helped them build up functional reserves in the body,and the preoperative kinesitherapy for prehabilitation was effective in improving patients'cardia and pulmonary endurance.According to the nurses,the clinic provided patients with diverse preoperative nursing service,and elevated nurses'values.Conclusions The specialized nursing clinic can provide patients with diverse preoperative nursing service for prehabilitation,help them build up functional reserves in their body,and improve their endurance of cardiac and pulmonary function.Therefore,it is conductive to better postoperative rehabilitation.
