呼吸湿化治疗仪治疗老年轻中度OSAHS患者的临床效果分析

摘　　要：目的探讨呼吸湿化仪对老年轻、中度阻塞性呼吸暂停低通气综合征(OSAHS)患者的疗效。方法以2017年10月至2019年4月于温州老年病医院综合病区确诊的30例老年轻、中度OSAHS患者为研究对象随机将患者分为治疗组(15例)与对照组(15例),治疗组在睡眠时给予呼吸湿化治疗仪经鼻高流量氧疗并给予常规治疗。对照组在睡眠时应用常规鼻导管氧疗并给予常规治疗,总干预时间均为2个月。观察并比较两组在干预前后呼吸紊乱指数(AHI)、最低血氧饱和度(LSaCh)、Epworth嗜睡量表评分、以及白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)及正五聚蛋白(PTX-3)的变化。结果治疗组和对照组干预后的AHI、ESS、IL-6、TNF-α、PTX-3均低于干预前,LSaO2高于干预前,差异有统计学意义(P<0.05);干预后,治疗组的AHI、ESS、IL-6、TNF-α、PTX-3均低于对照组,LSaO2高于对照组,差异有统计学意义(P<0.05)。结论呼吸湿化仪对老年轻、中度OSAHS有一定的治疗作用,可抑制患者体内的IL-6、TNF-α、PTX-3水平。Objective To investigate the effect of respiratory humidifieron advanced—aged patients with mile-to-moderate OSAHS.Methods A total of 30 elderly mile—to—moderate OSAHS patients were enroDed in the study.There were randomly divided into treatment group and control group(n=15,each).The treatment group received respiratory humidifier with high-flow nasal cannula combined regular treatment.The control group received nasal catheter cannula combined regular treatment.The total intervention time was 2 months.Difference in Apnea Hypopnea Index(AHI),the lowest SaC>2(LSaC>2),Epworth sleepiness score(ESS),serum interleukin—6(IL—6)levels,serum tumor necrosis factor—α(TNF—α)and Pentraxin 3(PTX—3)levels between the two groups were compared.Results Compared with before intervention,AHI,ESS scores,IL—6 levels,TNF—αand PTX-3 levels were significantly decreased and LSaCh were significantly increased in both group(P<0.05).After treatment,AHI,ESS scores,IL~6 levels,TNF—αand PTX—3 levels in treatment group were significantly lower than those control group(P<0.05).Treatment group patients had significantly higher LSaC>2 than control group(P<0.05).Conclusion Respiratory humidifier has a positive effect on elderly mile-tomoderate OSAHS patients,which reduces the level of IL-6,TNF-αand PTX-3.
