摘　　要：目的对比分析改良舌腭弓"L"形切口与传统口咽入路在治疗茎突过长综合征的效果。方法回顾分析2000~2018年我科诊治的34例茎突过长综合征患者,根据手术方式分为传统口咽入路组18例(A组)和改良舌腭弓"L"形切口组16例(B组)。对比分析两组手术时间、术中出血量、术后住院天数、术后患侧茎突长度和并发症情况。结果 B组在手术时间(t=0.247)、术中出血量(t=0.206)、术后住院天数(t=0.322)均小于A组,差异有统计学意义(P<0.05);A、B两组术后患侧茎突长度均在正常范围内,差异无统计学意义(t=1.215,P>0.05);短期随访两组均未见严重手术并发症。结论改良舌腭弓"L"形切口可以有效截短茎突,较传统口咽入路更有利于患者康复,值得临床推广应用。OBJECTIVE To explore the effect of improved L-shape incision of palatoglossal arch on the treatment of Eagle’s syndrome in contrast with traditional oropharyngeal approach.METHODS From January 2000 to December 2018,34 patients with Eagle’s syndrome were selected:18 patients were treated with the traditional oropharyngeal approach(Group A),16 patients were treated with improved L-shape incision of palatoglossal arch(group B).Then the operative time,intraoperative blood loss,postoperative hospitalization days,length of styloid process and complications were compared and analyzed between the two groups.RESULTS The operative time(t=0.247),intraoperative blood loss(t=0.206) and postoperative hospitalization days(t=0.322) in group B were all fewer than those in group A(P<0.05).The length of affected styloid process after surgery is not statistic significant between the two groups(t=1.215,P>0.05).No serious surgical complications occur in both group during short-term follow-up.CONCLUSION The improved L-shape incision of palatoglossal arch is more beneficial to patient’s recovery than the traditional oropharyngeal approach.It could be employed for the patient with Eagle’s syndrome to remove the long styloid process effectively.
