多指指尖离断再植12例临床体会

摘　　要：目的探讨多指指尖离断再植的方法选择与临床效果。方法2016年5月-2018年11月,对12例多指指尖离断病例,根据离断类型,采用仅吻合动脉、动脉静脉化等方法再植。结果共再植12例31指,成活30指,坏死1指。术后随访6~12个月,平均8个月,2例拔甲患指指甲稍有畸形,余再植指体指甲生长良好、无畸形,指腹饱满,痛、温觉基本恢复,两点辨别觉4~5 mm。结论对于多指指尖离断,根据离断类型,灵活选用仅吻合动脉、动脉静脉化等方法再植,可取得满意的临床效果。Objective To discuss the method selection and clinical effect of multi-finger replantation.Methods From May 2016 to November 2018,12 cases of multiple fingertip amputation were replanted by methods such as arterial anastomosis alone and arterial venation according to the type of amputation.Results A total of 31 fingers were replanted in 12 cases,30 fingers survived and 1 finger was necrotic.The postoperative follow-up was 6 to 12 months,with an average of 8 months.Two cases of nail removal had slight deformations.The rest of the replanted finger nails grew well without deformities,the finger belly was full,the pain and temperature sensation basically recovered,and the two-point discrimination was 4 to 5 mm.Conclusion For multi-finger fingertip amputation,according to the type of dissection,methods such as arterial anastomosis and arterial venation can be flexibly selected to achieve satisfactory clinical results.
