应用注射器自制负压吸引装置治疗掌指骨慢性骨髓炎

摘　　要：目的探讨掌指骨慢性骨髓炎清创术后的引流方法。方法2007年1月-2018年12月,对66例掌指骨慢性骨髓炎在清创后应用一次性注射器自制的负压吸引装置引流,并观察治疗效果。结果66例均获得随访,随访时间6~21个月,平均8.4个月。54例切口一期愈合,无感染反复及创口不愈合现象;12例创缘缺损或坏死经换药治疗瘢痕愈合;66例骨髓炎全部控制,随访半年以上无复发,23例骨折未愈合或骨缺损骨髓炎治愈半年后行二期植骨愈合。结论掌指骨慢性骨髓炎应用一次性注射器自制的负压吸引装置治疗,方法便捷、简单,费用低廉,疗效满意。Objective To explore the clinical curative effect of focal debridement combined with the disposal syringe negative pressure suction in the treatment of patients with chronic osteomyelitis of metacarpal bones and digital phalanges.Methods From January 2007 to December 2018,66 cases of patients with chronic osteomyelitis of metacarpal bones and digital phalanges treated by focal debridement combined with the disposal syringe negative pressure suction.Observed the clinical curative and prognosis after treatment in the patients.Results Sixty-six cases were followed-up for 2~18 months(average time of 6.3 months).No recurrence had happended among the 66 cases after treatment.There were 54 cases of primary healing of incision.Although 12 cases of skin flap necrosis after operative,which were successfully treated with dressing.23 patients with bone defect and bone nonunion were successfully treated by bone graft after 6 months.Conclusion Patients with chronic osteomyelitis of metacarpal bones and digital phalanges can be treated by focal debridement combined with the disposal syringe negative pressure suction,the clinical curative effect is remarkable.Moreover,simple in operation and low in costs,it is worth promoting.
