变应性鼻炎就医行为的影响因素分析

摘　　要：目的分析门诊变应性鼻炎(AR)患者就医行为的影响因素。方法收集2018年1月~2019年1月于我院就诊的AR患者,给患者提供治疗方案和药物治疗,并进行患者教育,嘱咐患者定期随访。在此期间预约患者复诊,在半年后调查没有按时随访复诊的患者和按时随访复诊的患者的资料,比较两组患者人群的基本情况。采用多因素logistic回归分析AR患者按时随访复诊的影响因素。结果共计160名患者就诊,随访半年后按时复诊的96例(60.0%),不按时复诊的64例(40.0%)。单因素分析:患者的学历、经济水平、医药费承受能力、病情严重程度、对医疗服务的满意程度、对医护人员的信任程度在两组间比较差异有统计学意义(P<0.05),多因素分析:低学历、高收入、疾病严重程度较低、对医疗服务的满意度较低是AR患者不按时复诊的独立危险因素(P<0.05)。结论对于受教育程度低、高收入工作忙、疾病严重程度较低的人群,合理用药,合理检查,改善服务态度,调整服务方式,对提高患者的按时复诊行为,对预防AR并发症具有积极的意义。Objective To analyze the influencing factors of health seeking behavior of patients with allergic rhinitis.Methods We investigated AR patients treated in our hospital from January 2018 to December 2018,and conducted drug treatment and patient education.Questionnaires were used to record patient-related information and patients were informed that they need to be revisited every two weeks to March.Follow-up was conducted six months later to investigate patients'follow-up visits and treatment compliance.A total of 162 patients who completed the survey were divided into the active medical group(n=75)and the inactive medical group(n=87)according to whether the patients could actively seek medical treatment.Multivariate logistic regression was used to analyze the influencing factors of inactive behaviors in AR patients.Results A total of 162 patients completed the survey,of which 75 were in the active medical group and 87 were inactive.The analysis shows that age is less than 18 years,occupation is student or farmer or no occupation,economic income is less than 3,000 yuan/month,living environment is rural,education level is below junior high school,negative attitude,and unclear AR complications is AR Independent risk factors for patients not actively seeking medical treatment(P<0.05).Conclusion Low economic income,rural residents,low education level,student population,and low level of understanding of the disease are independent risk factors for patients with allergic rhinitis who do not seek medical treatment.
