摘  要:目的探讨肺炎支原体肺炎(MMP)伴喘息患儿血清核转录因子-κB(NF-κB)、高迁移率族蛋白B1(HMGB1)表达水平与炎性细胞因子的相关性及其诊断价值。方法选取2018年1月至2019年10月该院收治的MMP患儿90例,分为MMP伴喘息组38例,MMP不伴喘息组52例,另选取同期体检健康儿童40例作为健康对照组,应用荧光定量PCR检测各组血清NF-κB、HMGB1表达水平,应用酶联免疫吸附试验法检测各组血清白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)和肿瘤坏死因子-α(TNF-α)水平,并进行Pearson相关分析,利用受试者工作特征(ROC)曲线分析血清NF-κB、HMGB1表达对MMP伴喘息的诊断价值。结果MMP伴喘息组与MMP不伴喘息组患儿血清NF-κB、HMGB1表达水平与IL-1β、IL-6和TNF-α水平均高于健康对照组,差异有统计学意义(P<0.05);MMP伴喘息组患儿血清NF-κB、HMGB1表达水平与IL-1β、IL-6和TNF-α水平均高于MMP不伴喘息组,差异有统计学意义(P<0.05)。MMP伴喘息患儿血清NF-κB表达水平与血清IL-1β、IL-6及TNF-α水平呈正相关,血清HMGB1表达水平与血清IL-1β、IL-6及TNF-α水平呈正相关。血清NF-κB的最佳临界值为4.78,ROC曲线下面积(AUC)为0.863(95%CI:0.752~0.895),其灵敏度为88.28%,特异度为87.38%;血清HMGB1的最佳临界值为3.67,AUC为0.815(95%CI:0.746~0.895),其灵敏度为84.74%,特异度为80.43%,诊断价值均优于血清IL-1β、IL-6及TNF-α。结论NF-κB、HMGB1、IL-1β、IL-6及TNF-α可能共同参与了MMP伴喘息的病理发展过程,血清NF-κB、HMGB1表达对MMP伴喘息患儿具有较好的诊断价值。Objective To investigate the correlation between the expression of nuclear factor-kappa B(NF-κB)and high mobility group protein B1(HMGB1)in serum and inflammatory cytokines in children with mycoplasma pneumoniae pneumonia(MMP)and wheezing and its diagnostic value.Methods Totally 90 children with MMP from January 2018 to October 2019 in our hospital were selected,which were divided into MMP with wheezing group(38 cases),MMP without wheezing group(52 cases),40 healthy children in the same period were selected as the control group.The levels of NF-κB and HMGB1 expression in each group were detected by fluorescence quantitative PCR,and the levels of interleukin-1β(IL-1β),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)were detected by enzyme-linked immunosorbent assay,and the Pearson correlation analysis were use to analyze their relevance.The expression of NF-κB and HMGB1 in serum were analyzed by receiver operating characteristic(ROC)curve to diagnose MMP with wheezing.Results The levels of serum NF-κB,HMGB1,IL-1β,IL-6 and TNF-αin MMP with wheezing group and MMP without wheezing group were significantly higher than those in the healthy control group,the difference was statistically significant(P<0.05).The serum levels of NF-κB,HMGB1,IL-1β,IL-6 and TNF-αwere significantly higher in MMP with wheezing group than in MMP without wheezing group,the difference was statistically significant(P<0.05).The expression of NF-κB was positively correlated with the levels of IL-1β,IL-6 and TNF-α.The expression of HMGB1 was positively correlated with the levels of IL-1β,IL-6 and TNF-α.The best critical value of serum NF-κB was 4.78,the area under the ROC curve(AUC)was 0.863(95%CI:0.752—0.895),the sensitivity was 88.28%and the specificity was 87.38%.The best critical value of serum HMGB1 was 3.67,the AUC was 0.815(95%CI:0.746—0.895),the sensitivity was 84.74%and the specificity was 80.43%,the diagnostic value of serum NF-κB and HMGB1 for MMP with wheezing was better than that of serum IL-1β,IL-6 and TNF-
