血清Occludin、MMP-2在间质性肺炎中的表达及其对预后的影响研究

摘　　要：目的探讨咬合蛋白(Occludin)和基质金属蛋白酶-2(MMP-2)在特发性间质性肺炎(IIP)中的表达及其对预后的影响。方法选取2014年8月至2019年8月该院收治的IIP患者79例作为IIP组,同时选取体检健康者82例作为对照组。比较各组血清Occludin、MMP-2水平及肺功能、血气相关指标,同时分析血清Occludin、MMP-2水平与IIP患者预后相关性。结果IIP组血清Occludin水平明显低于对照组,而MMP-2、肿瘤坏死因子α(TNF-α)水平及胸部高分辨CT(HRCT)评分明显高于对照组,差异均有统计学意义(P<0.05)。IIP组中,血清Occludin水平与MMP-2水平呈负相关(P<0.001);血清Occludin水平与TNF-α、C反应蛋白(CRP)水平及HRCT评分呈负相关(P<0.05),而血清MMP-2水平与这些指标呈正相关(P<0.05)。Occludin、MMP-2、TNF-α、CRP、第1秒用力呼气容积、血氧分压、一氧化碳测定肺弥散量、肺总量、白细胞计数、动脉血二氧化碳分压及HRCT评分是影响IIP患者不良预后的独立危险因素(P<0.05)。血清Occludin、MMP-2联合检测预测IIP患者不良预后的曲线下面积为0.861,灵敏度为89.50%,特异度为80.50%。结论血清Occludin、MMP-2水平与IIP患者不良预后密切相关,二者联合检测对预测IIP患者死亡风险有一定价值。Objective To investigate the expression of Occludin and matrix metalloproteinase-2(MMP-2)in idiopathic interstitial pneumonia(IIP)and its influence on prognosis.Methods 79 patients with IIP in the hospital from August 2014 to August 2019 were selected as IIP group,and 82 healthy people were selected as control group.The levels of serum Occludin,MMP-2,and the related indexes of lung function and blood gas were compared between the each group.The correlation between the levels of serum Occludin and MMP-2 and the prognosis of patients with IIP was analyzed.Results The levels of serum Occludin in the IIP group were significantly lower than those in the control group,while the levels of MMP-2,tumor necrosis factorα(TNF-α)and chest high resolution CT(HRCT)scores in IIP group were significantly higher than those in the control group(P<0.05).In the IIP group,serum Occludin level was negatively correlated with MMP-2 level(P<0.001);serum Occludin level was negatively correlated with TNF-α,C-reactive protein(CRP)level and HRCT score(P<0.05),while serum MMP-2 level was positively correlated with these indexes(P<0.05).Occludin,MMP-2,TNF-α,CRP,forced expiratory volume in the first second,blood oxygen partial pressure,carbon monoxide measurement of lung diffusion capacity,total lung volume,white blood cell count,arterial carbon dioxide partial pressure and HRCT score were independent risk factors for poor prognosis of IIP patients(P<0.05).The area under the curve of combined detection of serum Occludin and MMP-2 in predicting the poor prognosis of IIP patients was 0.861,the sensitivity was 89.50%,and the specificity was 80.50%.Conclusion The serum levels of Occludin and MMP-2 are closely related to the poor prognosis of patients with IIP.The combined detection of them has a certain value in predicting the death risk of patients with IIP.
