摘  要:目的:探讨胆固醇酯转移蛋白(CETP)活性水平的性别差异及其与心血管病危险因素的关系。方法:以参加"中国多省市心血管病队列研究"2002年危险因素调查的,北京地区45~74岁队列人群中资料完整的1350人(男性627人,女性723人)作为研究对象。CETP活性水平采用荧光法测定。对男女两性的CETP活性水平以及CETP活性与危险因素的关系进行分析。结果:总人群中CETP活性水平为83.19(66.25~103.58)pmol·μL^(-1)·h^(-1),女性高于男性[86.44(69.56~106.75)vs.79.91(62.78~100.29)pmol·μL^(-1)·h^(-1),P=0.001]。(2)男性年龄、血压以及饮酒、超重和高血压患病率高于女性,而女性的TG、HDL-C高于男性。(3)相关分析显示,男女两性的CETP活性与BMI、血脂(TG、HDL-C)及hs-CRP均相关(P<0.01)。女性CETP活性与LDL-C水平呈负相关(P=0.014),而男性CETP活性与载脂蛋白B-100呈正相关(P=0.010)。(4)多因素Logistic回归分析显示,在调整年龄、BMI、收缩压、LDL-C、HDL-C、TG、FBG、hs-CRP、吸烟、饮酒、服用降脂药后,女性CETP活性水平仍高于男性(OR=1.863,95%CI:1.354~2.563,P=0.001)。结论:女性CETP活性水平高于男性,性别对CETP活性水平可能具有独立影响。Objective:This study was designed to explore the gender-specific difference in cholestery ester transfer protein(CETP)activity and its association with known cardiovascular risk factors.Methods:Participants in this study were recruited from the Beijing project in the Chinese multi-provincial cohort study(CMCS).A total of 1350 participants with complete data were finally included,with 627 males and 723 females.CETP activity was determined by the BioVision’s CETP Activity Fluorometric Assay Kit.Analysis was done on the difference in CETP activity between genders and its association with known cardiovascular risk factors.Results:The CETP activity in the total participants was 83.19(66.25-103.58)pmol·μL^(-1)·h^(-1),and it was higher in females than in males[86.44(69.56-106.75)vs.79.91(62.78-100.29)pmol·μL^(-1)·h-1,P=0.001].(2)The age,blood pressure,alcohol,overweight,and prevalence of hypertension are higher in males,while TC and HDL-C are higher in females.(3)Correlation analysis showed that CETP activity was positively correlated with BMI,triglycerides,hs-CRP,and negatively correlated with HDL-C in both genders(P<0.01).The CETP activity was negative correlated with LDL-C in females,yet positively correlated with apolipoprotein B-100 in males(P<0.05).(4)After adjustment for age,BMI,SBP,LDL-C,HDL-C,triglycerides,blood glucose,hs-CRP,smoking,drinking,and lipid-lowering drugs in a multiple logistic regression model,CETP activity was still significantly higher in females than in males(OR=1.863,95%CI:1.354-2.563,P=0.001).Conclusions:CETP activity was higher in females than in males.But this gender-specific difference cannot be fully explained by the gender difference of the major cardiovascular risk factors.
