新型股骨近端内固定系统内固定术在股骨粗隆间骨折治疗中的应用效果及可行性分析

摘　　要：目的探究新型股骨近端内固定系统(Proximal Femoral Nail Antirotation,PFNA)内固定术在股骨粗隆间骨折治疗中的应用效果及可行性。方法选取太仓市浏河人民医院2015年6月至2020年1月收治的40例股骨粗隆间骨折患者为研究对象。采用计算机完全随机分组法将患者分为试验组与参照组,各20例。试验组患者采用PFNA内固定术进行治疗。参照组患者采用髋动力加压钢板(Dynamic Hip Screw Plate,DHS)内固定术进行治疗。对比两组患者的手术相关指标、髋功能评分及术后并发症发生情况。结果试验组患者的手术时长、术中出血量、住院时间、愈合时间均显著少于参照组患者,差异有统计学意义(t=2.995、2.118、2.244、5.211,P<0.05)。试验组患者的髋功能优良率为95.00%,参照组患者的髋功能优良率为70.00%,试验组患者术后6个月的髋功能明显好于参照组患者,差异有统计学意义(χ^(2)=4.329,P<0.05)。试验组患者的术后并发症率为10.00%,参照组患者的并发症率为40.00%,试验组患者的术后并发症出现情况显著少于参照组患者,差异有统计学意义(χ^(2)=4.800,P<0.05)。结论PFNA内固定术在股骨粗隆间骨折治疗中的应用效果较好,对比DHS内固定术,患者的手术指标及髋功能有明显提高,患者的术后并发症显著降低。Objective To explore the effect and feasibility of PFNA internal fixation in the treatment of intertrochanteric fracture.Methods A total of 40 cases of intertrochanteric fracture to Liuhe People's Hospital of Taicang City from June 2015 to January 2020 were selected as the research subjects.The patients were randomly divided into experimental group and control group,20 cases in each group.Patients in the experimental group were treated with PFNA internal fixation.The control group was treated with DHS internal fixation.The operation related indexes,hip function score and postoperative complications of the two groups were compared.Results The operation time,intraoperative blood loss,hospitalization time and healing time of the experimental group were significantly less than those of the control group(t=2.995,2.118,2.244,5.211,P<0.05).The excellent and good rate of hip function in the experimental group was 95.00%,and that in the control group was 70.00%.The hip function of the experimental group was significantly better than that of the control group at 6 months after operation(χ^(2)=4.329,P<0.05).The postoperative complication rate of the experimental group was 10.00%,and that of the control group was 40.00%.The incidence of postoperative complications in the experimental group was significantly less than that in the control group(χ^(2)=4.800,P<0.05).Conclusion PFNA internal fixation in the treatment of femoral intertrochanteric fracture has a good effect.Compared with DHS internal fixation,the operation indexes and hip function of patients are significantly improved,and the postoperative complications of patients are significantly reduced.
