椎体后凸成形术治疗骨质疏松性脊柱骨折的价值体会

摘　　要：目的讨论椎体后凸成形术与保守治疗骨质疏松性脊柱骨折的价值。方法选取2014年10月至2019年11月昆山市锦溪人民医院进行治疗的116例骨质疏松性脊柱骨折患者为研究对象,按照随机数字表法分为对照组和研究组,每组58例。对照组实施保守疗法,研究组实施椎体后凸成形术;观察疗效、视觉疼痛模拟评分(VAS)、椎体高度、Cobb角、并发症发生率及生活质量。结果研究组治疗有效率为94.83%,对照组治疗有效率为79.31%,相比统计学差异明显(P<0.05)。治疗前患者的VAS评分、椎体高度及Cobb角指标无明显差异(P>0.05);治疗后研究组VAS评分、椎体高度及Cobb角指标明显优于对照组(P<0.05)。研究组并发症发生率为1.72%低于对照组的17.24%,差异有统计学意义(P<0.05)。治疗前生活质量评分无明显差异(P>0.05);治疗后研究组生活质量评分优于对照组(P<0.05)。结论以椎体后凸成形术治疗骨质疏松性脊柱骨折病症的疗效显著,可促进患者病情恢复,缓解疼痛感,降低并发症发生率,提高生活质量。Objective To discuss the value of kyphoplasty and conservative treatment of osteoporotic spinal fractures.Methods From October 2014 to November 2019,116 patients with osteoporotic spinal fracture treated by Jinxi People's Hospital of Kunshan were selected as the subjects.According to the random number table method,they were divided into control group and study group,58 cases in each group.The control group was treated with conservative therapy and the study group was treated with kyphoplasty.The curative effect,visual pain simulation score(VAS),vertebral height,Cobb angle,complication rate and quality of life were observed.Results The effective rate of the study group was 94.83%,and that of the control group was 79.31%,which was significantly different(P<0.05).There was no significant difference in VAS score,vertebral height and Cobb angle before treatment(P>0.05);after treatment,the VAS score,vertebral height and Cobb angle of the study group were significantly better than those of the control group(P<0.05).The incidence of complications in the study group was 1.72%,lower than that in the control group 17.24%,the difference was statistically significant(P<0.05).There was no significant difference in quality of life score before treatment(P>0.05);after treatment,the score of quality of life in the study group was better than that in the control group(P<0.05).Conclusion Kyphoplasty is effective in the treatment of osteoporotic spinal fracture,which can promote the recovery of patients'condition,relieve pain,reduce the incidence of complications and improve the quality of life.
