摘　　要：目的分析B超联合腔内心电图技术(IEGM)定位早产儿经外周静脉穿刺中心静脉置管(PICC)的效果。方法选取2018年4月至2019年12月在郑州市妇幼保健院行PICC置管的早产儿74例按照交替分组法分为对照组与观察组,每组37例。对照组接受B超引导下PICC置管,观察组接受B超联合IEGM定位PICC置管。观察两组置管后成功率,记录并比较两组导管异位情况及非计划拔管情况。结果对照组37例患儿中,PICC置管成功27例(72.97%);观察组37例患儿中,PICC置管成功36例(97.30%)。观察组PICC置管的成功率明显高于对照组,差异有统计学意义(χ^(2)=8.649,P=0.003)。观察组非计划拔管与导管异位的总发生率均较对照组低,差异有统计学意义(均P<0.05)。结论B超联合IEGM定位早产儿PICC置管的效果较佳,置管成功率较高,且非计划拔管与导管异位的发生率较低。Objective To analyze the effect of B-ultrasound combined with intracavitary electroencephalography(IEGM)in locating peripherally inserted central catheter(PICC)in premature infants.Methods Seventy-four preterm infants who underwent PICC catheterization in Zhengzhou Maternal and Child Health Hospital from April 2018 to December 2019 were selected and divided into control group and observation group according to the alternate grouping method,with 37 cases in each group.The control group received PICC catheterization guided by B-ultrasound,while the observation group received PICC catheterization guided by B-ultrasound combined with IEGM.The success rate of catheterization in the two groups was observed,and the situation of ectopic catheter and unplanned extubation were recorded and compared.Results In the control group,27 cases(72.97%)were successful in PICC catheterization,and 36 cases(97.30%)in the observation group.The success rate of PICC catheterization in the observation group was significantly higher than that in the control group(χ^(2)=8.649,P=0.003).The total incidences of unplanned extubation and ectopic catheter in the observation group were lower than those in the control group,and the differences were significant(P<0.05).Conclusions B-ultrasound combined with iegm has better effect on PICC catheterization in preterm infants,and the success rate of catheterization is higher,and the incidence of unplanned extubation and ectopic catheter is lower.
