摘  要:目的探讨革兰阳性菌(G^(+)菌)、革兰阴性菌(G^(-)菌)感染与脓毒症患者病情程度的关系。方法回顾分析2018年1月至2019年12月三门峡市中心医院收治的108例脓毒症患者的临床资料。评估患者的病情程度,包括早期脓毒症、重度脓毒症、脓毒症休克;统计患者的一般资料,包括性别、年龄、发病至转重症监护室(ICU)时间、感染部位、住院原因、合并糖尿病情况、菌群种类(G^(+)菌、G^(-)菌)。分析不同菌群感染与脓毒症患者病情程度的相关性。结果 108例患者中,早期脓毒症59例(54.63%),重度脓毒症30例(27.78%),脓毒症休克19例(17.59%)。不同病情程度患者的性别、年龄、发病至转ICU时间、感染部位、住院原因、合并糖尿病情况组间比较,差异未见统计学意义(P>0.05);菌群种类组间比较差异有统计学意义(P<0.05)。经二元Logistic回归分析后建立多元回归模型发现,G^(-)菌感染是脓毒症患者发生休克的影响因素(OR>1,P<0.05)。结论与G^(+)菌感染相比,G^(-)菌感染的脓毒症患者病情更为严重,发生休克的风险也更高。Objective To investigate the relationship of Gram-positive bacteria(G^(+)bacteria)and Gram-negative bacteria(G^(-)bacteria)infection with severity of sepsis.Methods The clinical data of 108 patients with sepsis treated in Sanmenxia Central Hospital from January 2018 to December 2019 were retrospectively analyzed.The patients’condition was assessed,including early sepsis,severe sepsis,and septic shock.The general information of patients were counted,including gender,age,time from onset to intensive care unit(ICU)admission,site of infection,reason of hospitalization,comorbidity of diabetes,and type of flora(G^(+)bacteria,G^(-)bacteria).The relationship between the infection of different flora with the severity of sepsis were analyzed.Results Among the 108 patients with sepsis,59 cases(54.63%)were early sepsis,30 cases(27.78%)were severe sepsis,and 19 cases(17.59%)were septic shock.There was no significant difference in gender,age,time from onset to ICU admission,site of infection,reason for hospitalization,and comorbidity of diabetes among patients with different disease severity(P>0.05).However,there were significant differences in the above indexes among groups with different bacterial species(P<0.05).After binary Logistic regression analysis,a multiple regression model was established,and it was found that G-bacteria infection was an influencing factor for shock in patients with sepsis(OR>1,P<0.05).Conclusions Compared with G+bacteria infection,patients with sepsis infected by G-bacteria have more severely illness,and have a higher risk of shock.
