复方麝香注射液联合神经节苷脂治疗新生儿缺氧缺血性脑病的效果观察

摘　　要：目的探讨复方麝香注射液联合神经节苷脂对新生儿缺氧缺血性脑病(HIE)的疗效。方法抽取2017年3月至2020年4月在郑州市妇幼保健院接受治疗的新生儿HIE患儿80例作为研究对象,按照随机数字表法分为观察组和对照组,每组40例,对照组给予神经节苷脂治疗,观察组给予复方麝香注射液联合神经节苷脂治疗。比较两组临床症状改善情况、近期疗效、血清神经肽Y(NPY)、神经元特异性烯醇化酶(NSE)、β-内啡肽(β-EP)水平、新生儿行为神经测定评分(NBNA)及治疗过程中的不良反应。结果治疗后,观察组反射、意识、肌张力恢复时间短于对照组(P<0.05)。观察组治疗总有效率(92.50%,37/40)高于对照组(75.00%,30/40),差异有统计学意义(P<0.05)。治疗前,两组NPY、NSE、β-EP水平比较差异未见统计学意义(P>0.05);治疗后,两组NPY、NSE、β-EP水平均降低,且观察组下降幅度大于对照组(P<0.05)。治疗前,两组NBNA评分比较差异未见统计学意义(P>0.05);治疗后,观察组NBNA评分高于对照组(P<0.05)。治疗过程中,两组均未发生严重不良反应。结论复方麝香注射液联合神经节苷脂治疗新生儿HIE疗效较好,可改善患儿临床症状,降低血清NPY、NSE、β-EP水平,减少对神经功能的损伤,药物安全性高。Objective To investigate the effects of compound musk injection combined with gangliosides on neonates with hypoxic-ischemic encephalopathy(HIE).Methods A total of 80 neonates with HIE treated in Zhengzhou Maternal and Child Health Hospital from March 2017 to April 2020 were selected as the research subjects.They were divided into observation group and control group by random number table method,with 40 cases in each group.The control group was treated with gangliosides,and the observation group was treated with compound musk injection combined with gangliosides.The improvement of clinical symptoms,short-term curative effect,levels of serum neuropeptide Y(NPY),neuron specific enolase(NSE),β-endorphins neonatal(β-EP),neonatal behavioral neurological assessment(NBNA)scores,and incidence of adverse reactions during treatment were compared between the two groups.Results The recovery time of reflex,consciousness,and muscle tone of the observation group was significantly shorter than that of the control group(P<0.05).The total effective rate of treatment in the observation group(92.50%,37/40)was significantly higher than that in the control group(75.00%,30/40),P<0.05.There was no significant difference in levels of NPY,NSE orβ-EP between the two groups before treatment(P>0.05),but these indexes all decreased significantly after treatment in both groups(P<0.05);moreover,decreases of these indexes in the observation group were significantly larger than those in the control group(P<0.05).There was no significant difference in NBNA scores between the two groups before treatment(P>0.05),however,the score of the observation group was significantly higher than that of the control group after treatment(P<0.05).There was no serious adverse reaction in neither group during treatment.Conclusions Compound musk injection combined with gangliosides is effective in treatment of neonatal HIE.It can significantly improve the clinical symptoms of children,reduce levels of serum NPY,NSE andβ-EP,and alleviate the impact on nerve funct
