超声引导下前锯肌平面阻滞麻醉对开腹肝癌切除术患者疼痛程度及细胞免疫功能的影响

摘　　要：目的探讨超声引导下前锯肌平面阻滞麻醉(SAPB)在开腹肝癌切除术中的应用效果。方法将择期行开腹肝癌切除术的114例原发性肝癌患者采用随机抽签法分为对照组与观察组,各57例。对照组予以全麻,观察组予以超声引导下SAPB联合全麻。比较两组的麻醉效果。结果术后2、8 h,观察组的PHPS评分低于对照组(P<0.05)。观察组的舒芬太尼、瑞芬太尼用量及术后24、48 h内镇痛泵按压次数均少于对照组(P<0.05)。术后2、8 h,观察组的CD3^(+)、CD4^(+)/CD8^(+)及NK均高于对照组(P<0.05)。观察组的不良反应总发生率低于对照组(P<0.05)。结论超声引导下SAPB可减轻开腹肝癌切除术患者术后疼痛,减少阿片类药物用量,保护患者的细胞免疫功能,且安全性高。Objective To explore the application effect of ultrasound-guided serratus anterior plane block anesthesia(SAPB)in open hepatectomy.Methods A total of 114 patients with primary liver cancer who underwent elective open hepatectomy were divided into control group and observation group by the random drawing method,with 57 cases in each group.The control group was given general anesthesia,and the observation group was given ultrasound-guided SAPB combined with general anesthesia.The anesthetic effects of the two groups were compared.Results At 2 and 8 h after operation,PHPS scores in the observation group were lower than those in the control group(P<0.05).The dosage of sufentanil and remifentanil and the pressing times of analgesia pump in 24 and 48 h after operation in the observation group were less than those in the control group(P<0.05).At 2 and 8 h after operation,the CD3^(+),CD4^(+)/CD8^(+)and NK in the observation group were higher than those in the control group(P<0.05).The total incidence of adverse reactions in the observation group was lower than that in the control group(P<0.05).Conclusion Ultrasound-guided SAPB can reduce the postoperative pain of patients with open hepatectomy,reduce the dosage of opioids,and protect the cellular immune function of patients,with high safety.
