经皮腹壁单针道腹膜缝合在腹腔镜经腹膜前入路疝修补术中的应用

摘　　要：目的:探讨腹腔镜经腹膜前入路疝修补术中采用经皮腹壁单针道缝合腹膜法与腹腔内连续缝合腹膜法的临床疗效。方法:回顾性分析478例成人单侧腹股沟疝患者的临床资料,按手术方法不同分为穿刺组及缝合组,其中穿刺组240例,采用经皮腹壁单针道穿刺缝合关闭腹膜切口;缝合组238例,采用腹腔内操作连续缝合关闭腹膜切口,比较2组患者缝合腹膜时间、手术总时长、腹膜伤口长度及术后短期并发症等指标。结果:2组比较,穿刺组手术时间[(63.13±17.22)vs(70.66±23.71)min]、腹膜缝合时间[(6.66±2.63)vs(20.71±4.87)min]短于缝合组(P<0.05);穿刺组腹膜创面长度短于缝合组[(1.51+0.23)vs(10.45+2.05)cm],差异有统计学意义(P<0.05)。2组患者切口感染、局部渗血、中重度疼痛、神经感觉异常等并发症比较,差异无统计学意义(P>0.05)。结论:腹腔镜经腹膜前入路疝修补术采用经皮腹壁单针道缝合关闭腹膜方法,操作方便快捷,腹膜创面小。Objective:To evaluate the clinical outcome of the single-needle percutaneous peritoneum suture by comparing with the traditional intraperitoneal peritoneum suture in transabdominal preperitoneal prosthesis.Methods:A total of 478 adult patients with unilateral inguinal hernia admitted were reviewed and divided into puncture group and suture group according to different surgical methods.Among them,240 cases in puncture group were treated with single-needle percutaneous peritoneum suture;238 cases in suture group were treated with continuous suture in abdominal cavity.The duration of peritoneum suture,operation time,peritoneal wound length and short-term postoperative complications were analyzed.Results:The operation time and the peritoneal suture time were significantly shorter in the puncture group.Compared with the suture group,the length of peritoneal wound was much shorter in the puncture Group.There was no significant difference in incision infection,local bleeding,moderate to severe pain and abnormal nerve sensation between the two groups(P>0.05).Conclusion:Using single-needle percutaneous method closing peritoneum in laparoscopic hernia repair is a simple,convenient and time-saving method.
