山莨菪碱结合孟鲁司特治疗过敏性紫癜患儿的临床研究及复发的危险因素分析

摘　　要：目的探讨山莨菪碱结合孟鲁司特治疗过敏性紫癜的临床效果及复发的危险因素。方法将120例过敏性紫癜患者随机分组原则,随机分为观察组和对照组,每组60例,两组均采取孟鲁司特治疗,观察组患者联合采用山莨菪碱,均治疗4周。比较两组效果、症状改善、应激反应、炎性因子以及复发率差异,分析复发危险因素。结果观察组患者的总有效率(93.33%)显著高于对照组(73.33%);紫癜、腹痛、关节肿痛消失时间、IL-4、IL-10、INF-γ以及TNF-α水平显著低于对照组(P<0.05);应激反应相关激素水平低于对照组(P<0.05);观察组复发率显著低于对照组;反复性皮疹、EB病毒感染、较高血小板和D-二聚体、肾损害均为复发的危险因素。结论山莨菪碱结合孟鲁司特治疗过敏性紫癜者,炎性以及氧化应激反应均显著下降,复发率较低。Objective The aim of thisstudy was to explore the clinical effect of anisodamine combined with montelukast in the treatment of HSP.Methods A total of 120 patients with henoch schonlein purpura were randomly divided into observation group(60 cases)and control group(60 cases).Both groups were treated with montelukast,while patients in observation group were treated with Anisodamine for 4 weeks.The effect,symptom improvement,stress response,inflammatory factors and recurrence rate were compared between the two groups.Meanwhile,the recurrence risk factors were analyzed.Results The total effective rate of the observation group(93.33%)was significantly higher than that of the control group(73.33%).The disappearance time of purpura,abdominal pain,joint swelling and pain,the levels of IL-4,IL-10,inf-γand TNF-αwere significantly lower than those of the control group(P<0.05).In addition,the level of stress related hormones was lower than that of the control group(P<0.05)and the recurrence rate of the observation group was significantly lower than that of the control group.The recurrent rash,Epstein Barr virus infection and higher blood pressure were observed Small plate,D-dimer and renal damage were risk factors for recurrence.Conclusion In the treatment of allergic purpura by using Anisodamine combined with montelukast,the inflammatory and oxidative stress reactions were significantly decreasedand the recurrence was low.
