芪参益肺汤联合西药治疗肺癌放疗致放射性肺炎疗效观察

摘　　要：目的:研究对肺癌放疗致放射性肺炎采用中西药联合治疗的临床效果。方法:选取2015年1月-2018年11月我院收入治疗的肺癌患者68例,患者合并放疗致放射性肺炎疾病。将选取的研究对象按奇偶分类法分为对照组(34例)与观察组(34例),对两组患者采用西医常规治疗,在此基础上,对观察组行中西药联合治疗,中药治疗采用芪参益肺汤进行疾病干预。以4周为一个疗程,治疗3个疗程后,对比分析两组患者中医症状积分、血气分析及功能状态标准评分(采用卡氏评分)。结果:治疗前,两组患者主症、次症症状积分差异不显著,P>0.05;治疗后,观察组患者中医症状积分均显著低于对照组,差异显著(P<0.05)。治疗前,两组患者血气分析结果差异无统计学意义(P>0.05);治疗后,观察组患者动脉血二氧化碳分压(PaCO_(2))、血氧分压(PaO2)水平均显著优于对照组,差异具有统计学意义(P<0.05)。治疗前,两组患者肺功能指标差异无统计学意义(P>0.05);治疗后,观察组患者卡氏评分肺功能指标水平显著优于对照组,差异具有统计学意义(P<0.05);治疗前,对照组和观察组的白细胞介素(Interlerkin,IL)-6差异无统计学意义(P>0.05),治疗后,观察组患者的IL-6、肿瘤坏死因子-α(Tumor Necrosis Factor-α,TNF-α)水平均显著低于对照组,差异有统计学意义(P<0.05)。结论:对肺癌放疗致放射性肺炎采用中西药联合治疗,效果显著好于单纯采用西医常规治疗,方法具有推广意义。Objective:To study the clinical effects of the integrative medicine on radiation pneumonitis caused by radiotherapy of lung cancer.Methods:68 cases were divided into the control group(34 cases)and the observation group(34 cases)according to the odd-even classification method.The groups took conventional Western medicine.On this basis,the observation group was treated with the Qishen Yifei decoction(芪参益肺汤).A course of treatment lasted for 4 weeks,with 3 courses in total.Results:Before treatment,there was no significant difference in the scores of the primary and secondary syndromes of patients between the groups,P>0.05;after treatment,the above scores in the observation group were significantly lower,with significant difference(P<0.05).Before treatment,there was no significant difference in the results of blood gas analysis between the groups(P>0.05);after treatment,the PaCO_(2) and PaO2 in the observation group were significantly better,and the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference in pulmonary function indexes between the groups(P>0.05),after treatment,the Karnofsky score in the observation group was significantly better,P<0.05.Before treatment,there was no significant difference in IL-6 level between the groups(P>0.05),after treatment,the IL-6 and TNF-αlevels in the observation group were significantly lower than the control group,P<0.05.Conclusion:The integrative medicine on radiation pneumonia caused by radiotherapy of lung cancer is significantly better than conventional Western medicine alone,with promotion significance.
