摘 要:目的探讨甲状腺功能减退对妊娠晚期孕妇血糖、血脂水平及母婴结局的影响,以指导临床治疗,改善妊娠结局。方法回顾性分析2017年1月~2019年12月大连市妇幼保健院进行产前甲状腺功能检查的86例妊娠晚期孕妇的临床资料,依据甲状腺功能是否正常分为观察组(32例)和对照组(54例),观察组为甲状腺功能减退的患者,对照组为甲状腺功能正常者。比较两组的血糖、血脂相关指标水平及不良母婴结局发生情况。结果观察组空腹血糖、餐后1 h血糖、餐后2 h血糖、总胆固醇(TC)、三酰甘油(TG)水平均高于对照组,差异有统计学意义(P<0.05)。观察组不良母婴结局发生率高于对照组,差异有统计学意义(P<0.05)。结论甲状腺功能减退会提升妊娠晚期孕妇血糖、血脂水平,增加不良母婴结局的发生风险,故临床需及时采取干预措施,以改善母婴结局。Objective To investigate the effects of hypothyroidism on blood glucose,blood lipid levels,maternal and infant outcomes of pregnant women in late pregnancy,so as to guide clinical treatment and improve pregnancy outcomes.Methods The clinical data of 86 pregnant women in late pregnancy who underwent prenatal thyroid function examination in Dalian Maternal and Child Health Hospital from January 2017 to December 2019 were retrospectively analyzed,and according to the normal thyroid function,the patients were divided into the observation group(32 cases)and the control group(54 cases),patients with hypothyroidism were in the observation group and people with normal thyroid function were in the control group.The levels of blood glucose,blood lipid and maternal and the incidence of adverse maternal and child outcomes were compared between the two groups.Results The fasting blood glucose,1 h postprandial blood glucose,2 h postprandial blood glucose,total cholesterol(TC)and triglyceride(TG)levels in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The incidence of adverse maternal and child outcomes in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Hypothyroidism will increase the blood glucose and blood lipid levels of pregnant women in late pregnancy and increase the risk of adverse maternal and infant outcomes,so clinical interventions need to be taken in time to improve maternal and infant outcomes.
