摘 要:目的总结分析骨髓增生异常综合征(MDS)的临床特征及预后相关因素。方法收集40例MDS患者临床及实验室资料、骨髓病理、染色体核型、预后等信息,将以上信息进行统计学分析。结果 40例MDS患者平均发病年龄为(57.97±18.79)岁。所有就诊患者均有血象异常,三系均低患者最常见(50.0%),34例可见骨髓病态造血(85%),40例MDS患者染色体异常检出率为22.5%。按照IPSS标准对MDS患者进行预后分组,不同MDS预后分组患者与年龄差异无统计学意义(P>0.05),与白细胞、血红蛋白、血小板、不良染色体比例差异有统计学意义(P<0.05)。结论骨髓病态造血仍是MDS的诊断要点,染色体是MDS诊断的重要补充和预后分层的必要条件,依据不同分型给予患者个体化治疗,可改善其预后。Objective To analyze the pathogenesis and prognostic factors of primary myelodysplastic syndrome(MDS)and provide a reference for clinical treatment.Methods 40 MDS patients were selected and the clinical and laboratory data,bone marrow pathology,chromosome karyotype,prognosis and other information were collected and analyzed.Results The average age of onset in 40 patients with MDS was(57.97±18.79)years old.All patients had abnormal blood images,and the patients with low blood count of the three departments were the most common(50.0%).34 cases(85%)had morbid hematopoiesis of bone marrow.The detection rate of chromosomal abnormalities was 22.5%in 40 MDS patients.According to the IPSS standard,the prognosis group of MDS patients had no significant difference with age,but there was significant difference in the proportion of white blood cells,hemoglobin,platelets,and bad chromosomes was statistically significant.Conclusion Marrow pathological hematopoiesis is still the diagnostic point of MDS.Chromosome is an important complement to MDS diagnosis and a necessary condition for prognostic stratification.Individual treatment given to patients according to different types will improve the prognosis of patients.
