茵陈术附汤加减治疗肝衰竭并发难治性黄疸疗效研究

摘　　要：目的:观察茵陈术附汤加减治疗肝衰竭并发难治性黄疸临床疗效。方法:将收治的90例肝衰竭并发难治性黄疸患者随机分为两组。对照组45例患者采用临床常规治疗,观察组45例患者在对照组基础上加用茵陈术附汤加减治疗。治疗8周后,对比两组临床疗效、中医证候积分、肝功能指标、血清TBA、ChE、Alb指标、不良反应发生率,随访6个月,对比两组病死率。结果:观察组临床总有效率高于对照组(P<0.05)。观察组治疗后尿黄、腹胀、纳差、乏力症状积分低于对照组(均P<0.05)。观察组治疗后TBil、AST、ALT、TBA指标低于对照组(均P<0.05)。观察组治疗后血清PTA、ChE、Alb指标高于对照组(均P<0.05)。观察组病死率低于对照组(P<0.05)。观察组不良反应发生率与对照组比较,差异无统计学意义(P>0.05)。结论:茵陈术附汤可有效改善肝衰竭并发难治性黄疸患者肝功能,缓解临床症状,降低病死率,且不良反应较少。Objective:To analyze the effect of Yinchen Zhufu decoction in the treatment of liver failure complicated by refractory jaundice.Methods:90 patients with liver failure complicated by refractory jaundice were randomly divided into two groups.45 patients in the reference group were treated with clinical routine treatment,45 patients in the observation group were treated with Yinchen Zhufu decoction on the basis of the reference group,and both groups were treated for 8 weeks.Clinical effect,TCM syndrome score,liver function index,serum TBA,ChE,Alb of the two groups,incidence of adverse reactions were compared.The fatality rate of the two groups were compared after 6 months follow-up.Results:The total clinical effective rate of the observation group was significantly higher than that of the reference group(P<0.05).The scores of urine yellow,abdominal distention,poor appetite and asthenia symptoms in the observation group were significantly lower than those in the reference group(all P<0.05).The TBil,AST,ALT and TBA indexes in the observation group were significantly lower than those in the reference group(all P<0.05).The serum PTA,ChE and Alb indexes in the observation group were significantly higher than those in the reference group after treatment(all P<0.05).The fatality rate in the observation group was significantly lower than that in the reference group(P<0.05).The incidence of adverse reactions in the observation group was no statistical significance compared with that in the reference group(P>0.05).Conclusion:Yinchen Zhufu decoction can effectively improve the liver function of patients with liver failure complicated by refractory jaundice,alleviate clinical symptoms,reduce fatality rate,and fewer adverse reactions.
