富血小板纤维蛋白联合自体脂肪颗粒移植在面部年轻化中的应用研究

摘　　要：目的探讨富血小板纤维蛋白(platelet-rich fibrin,PRF)联合自体脂肪颗粒移植在面部年轻化中的应用效果。方法回顾性分析自2017年12月至2019年12月宜宾市第一人民医院整形烧伤科诊治的82例面部皮肤松垂或局部软组织缺失凹陷求美者的临床资料,按治疗方法分为观察组、对照组。两组均为双侧填充,对照组予以自体脂肪颗粒移植治疗,观察组予以PRF联合自体脂肪颗粒移植治疗。结果所有求美者术后获随访3~14个月,均未出现感染、血肿、硬结、囊肿、脂肪液化、色素沉着、感觉迟钝、局部凹陷等并发症。大部分求美者移植区较术前丰满、圆润,皮肤较术前紧致,富有弹性,局部凹陷明显改善,面部轮廓对称、协调。术后3个月,对照组填充脂肪被吸收有9例(22.5%)而行2次填充,观察组中有3例(7.1%)行2次填充,其差异具有统计学意义(P<0.05);观察组效果评价总有效率(83.3%)高于对照组(57.5%),P<0.05;观察组满意率(92.9%)高于对照组(72.5%),P<0.05。结论PRF联合自体脂肪颗粒移植应用于面部年轻化中效果明显,制备过程简单,术后并发症较少,且效果较持久,求美者的满意度较高。Objective To explore the efficacy of platelet-rich fibrin(PRF)combined with autologous fat transplantation in facial rejuvenation.Methods The clinical data of 82 beauty seekers with facial skin sagging or local depression from December 2017 to December 2019 were retrospectively analyzed.All beauty seekers were divided into the observation group and the control group according to the treatment method.Bilateral filling was performed in both groups.The beauty seekers in the control group were treated by autologous fat transplantation and the observation group were treated by PRF combined with autologous fat transplantation.Results All beauty seekers were followed up for 3 to 14 months.No complications such as infection,hematoma,induration,cyst,fat liquefaction,pigmentation,hypoesthesia and local depression occurred.Most patients had full transplantation area,tight and elastic skin.The local depression was significantly improved and the facial contour was symmetric and harmonious.Three months after operation,9 patients(22.5%)in the control group and 3 patients(7.1%)in the observation group were filled twice because of fat absorption.The difference was statistically significant(P<0.05).The total effective rate of the observation group(83.3%)evaluated by surgeons was higher than that of the control group(57.5%,P<0.05).The satisfaction rate of beauty seekers in the observation group(92.9%)was higher than that of the control group(72.5%,P<0.05).Conclusion PRF combined with autologous fat transplantation has obvious efficacy in facial rejuvenation.The preparation process is simple,the postoperative complications is less,the efficacy is long-lasting and patients’satisfaction is high.
