皮肤扩张术临床病例数据库的设计与应用

摘　　要：目的为了科学收集、整理皮肤扩张术临床病例资料,方便临床医师进行疗效评价和病例随访追踪而建立临床病例数据库。方法自2009年1月至2018年12月,共纳入空军军医大学第一附属医院(西京医院)整形外科行皮肤扩张术治疗的患者3140例。采用Access 2016软件建立包含患者围术期资料及随访记录的临床病例数据库。结果数据库具有系统登录、数据录入、病例查询、报表输出、资料链接等功能。经过筛选,最终纳入数据库的皮肤扩张术患者3053例(扩张器4266个),占总病例数的97.2%。患者中位年龄18岁,男性63.8%;烧伤后体表瘢痕占46.2%,外耳畸形占44.1%;一期扩张器置入术后并发症发生率占12.1%。结论本数据库具有简便易用、功能齐全的特点。初步分析发现,笔者科室近十年行皮肤扩张术的患者以青年男性为主,烧伤后体表瘢痕和外耳畸形是最常见的适应证,扩张器并发症发生率保持在较低水平。Objective To establish clinical case database for scientifically collect and collate clinical data of tissue expansion,facilitate the efficacy evaluation and follow-up.Methods A total of 3140 patients were enrolled in the Department of Plastic Surgery of The First Affiliated Hospital of Air Force Medical University from January 2009 to December 2018.We used Access 2016 software to establish a clinical case database containing patients’perioperative data and follow-up records.Results After establishment of the clinical case database,the database had functions such as system login,data entry,query function,report output and data link.After screening,3053 tissue expansion cases(4266 expanders)were finally included in the database,accounting for 97.2%of all consecutive cases.The median age of patients was 18 years old.Male patients accounted for 63.8%.Patients with scars after burns accounted for 46.2%,and deformation of external ear accounted for 44.1%.The complication rate after expander placement was 12.1%.Conclusion This database has characteristics such as easy to use and multiple functions.After preliminary analysis of the clinical data in the database,we find that the young male is the main population underwent tissue expansion in our department in the past 10 years.Scars after burns and deformation of external ear are the most common indications.Moreover,the incidence rate of expander complications remains relatively low.
