摘 要:目的观察56例瘢痕患儿采用皮肤镜和高频超声检查的图像特征。方法临床未经治疗的几类瘢痕患儿,每例皮损先后行皮肤镜及高频超声(频率50MHZ)检查,记录并统计各项图像特征。结果 56例瘢痕患者中,增生型瘢痕28例,皮肤镜下可见淡红(21例)或淡黄色背景(7例)、分支状血管(25例);高频超声下可见表皮变薄(24例)、真皮层增厚(28例)、回声不均(22例),可见线条状强回声(23例)。瘢痕疙瘩10例,皮肤镜下可见淡红色背景(9例)、网状血管(8例);高频超声下可见表皮变薄(7例)、真皮层增厚(10例),可见线条状强回声紊乱(8例)。浅表型瘢痕11例,皮肤镜下可见褐色色素沉着(10例)、分支状血管(8例);高频超声下表皮稍变薄(9例),真皮层基本正常(10例)。凹陷型瘢痕7例,皮肤镜下可见肤色背景(6例),中央无结构区(5例);高频超声下可见表皮正常(6例),真皮层萎缩(7例)。结论结合临床表现,皮肤镜和高频超声的图像特征可能为瘢痕的治疗及疗效评估提供帮助。Objective To describe high-frequency skin ultrasonographic and demoscopic features of children scars.Methods Children with several types of scar that are clinically untreated.One typical skin lesion was chosen from each patient and subjected to dermoscopy and high-frequency skin ultrasonographic,and imaging features were recorded and analyzed.Results Totally,56 untreated patients with scar were enrolled into this study,28 hypertrophic scars,dermoscopy revealed a light red background in 21 cases,a light yellow background in 7 cases,branching vessels in 25 cases.Ultrasonography showed,thinning of the skin in 24 cases,the thickening of the dermis in 28 cases,uneven echo in 22 cases,linear strong echo in 23 cases;10 keloids.Dermoscopy revealed a light red background in 9 cases,reticular vessels in 8 cases,ultrasonography showed thinning of the skin in 7 cases,the thickening of the dermis in 10 cases,linear hyperechoic disorder in 8 cases;11 superficial type of scars,dermoscopy revealed brown pigmentation in 10 cases,branching vessels in 8 cases;ultrasonography showed,thinning of the skin in 9 cases,the dermis was normal in 10 cases;7 pitting scars,dermoscopy revealed skin color background in 6 cases,central unstructured area in 5 cases;ultrasonography showed the epidermis is normal in 6 cases,atrophy of dermis in 7 cases.Conclusion In combination with clinical manifestations,the image features of dermoscopy and high-frequency ultrasonography may be helpful for the treatment and efficacy evaluation of scars.
