摘 要:目的:探讨高频超声在诊断婴幼儿先天性肥厚性幽门狭窄(CHPS)的临床应用价值。方法:回顾性分析我院收治的11例怀疑先天性肥厚性幽门狭窄患儿的超声表现,观察胃潴留情况、胃内容物通过幽门管情况、幽门管形态,测量幽门肌层厚度、幽门管直径、幽门管长度。结果:11例患儿均为足月儿,均为进行性呕吐加重,1例经内科治疗后好转,10例均经手术证实为CHPS,其中9例幽门肌厚度>4 mm,1例幽门肌厚度3 mm~4 mm。结论:超声具有安全简便无辐射的特点,能动态观察内容物通过幽门管情况,是检查CHPS的首选影像学检查方法和术后随访观察的最佳方法。Objective To investigate the diagnostic value of high-frequency ultrasound in congenital hypertrophic pyloric stenosis(CHPS).Methods The ultrasonographic manifestations of 11 children with suspected congenital hypertrophic pyloric stenosis admitted to our hospital were retrospectively analyzed to observe gastric retention,observe the morphology of pyloric tube through which gastric contents passed,and measure the thickness of pyloric muscle layer,diameter of pyloric tube and length of pyloric tube.Results All the 11 cases were full term infants with progressive vomiting,1 case improved after medical treatment,and all the 10 cases were confirmed as CHPS by surgery,among which 9 cases were>4mm thick and 1 case was 3 mm~4 mm thick.Conclusion Ultrasound is safe,simple and non-radiative,and can dynamically observe the contents passing through the pylorus tube,which is the preferred imaging examination method for CHPS examination and the best follow-up observation method.
