摘 要:目的:探讨超微血管成像技术(SMI)评估椎动脉起始段狭窄程度的可行性。方法:回顾性分析2018年10月—2020年3月在我院就诊患者132例,共264支血管,彩色多普勒超声(CDFI)检查提示椎动脉起始段狭窄或闭塞,并进行SMI检查及回访同一时期DSA的检查结果。将DSA检查结果作为金标准,分析CDFI、SMI评估椎动脉起始段狭窄程度的可行性。结果:SMI评估椎动脉起始段狭窄程度的结果与金标准具有高度一致性(Kappa=0.63,0.6<Kappa≤0.8,P<0.05),CDFI与金标准具有中度一致性(Kappa=0.49,0.4<Kappa≤0.6,P<0.05)。SMI评估椎动脉起始段狭窄程度的敏感性和准确率分别为88.7%、73.4%。结论:SMI能清晰显示椎动脉起始段血流情况,可以准确评估椎动脉起始段的狭窄程度,且优于彩色多普勒超声,分析得出SMI在评估椎动脉起始段狭窄程度中具有较高的可行性。Objective To explore the feasibility of superb microvascular imaging(SMI)in evaluating the stenosis degree of vertebral artery in the initial segment.Methods A retrospective analysis was conducted on 132 patients,264 vertebral arteries,who admitted to our hospital from October 2018 to March 2020.All patients were diagnosed with stenosis or occlusion in the the initial segment of vertebral artery by coloer Dopper ultrasound(CDFI).Then the patients were underwent SMI examination.The results of DSA in the same period were reviewed.DSA was used as the gold standard to analyze the feasibility of SMI and CDFI in evaluating the stenosis degree of vertebral artery.Results SMI is highly consistent with the gold standard in evaluating the stenosis degree of vertebral artery(Kappa=0.63,0.6<Kappa≤0.8,P<0.05),while CDFI is moderately consistent with the gold standard(Kappa=0.49,0.4<Kappa≤0.6,P<0.05).The diagnostic sensitivity and accuracy of SMI in evaluating the stenosis degree of vertebral artery were 88.7%、73.4%.Conclusion SMI can clearly display the blood flow of vertebral artery in the initial segment and accurately evaluate the stenosis degree of vertebral artery in the initial segment,which is better than color doppler ultrasound.It is concluded that SMI has a high feasibility in evaluating the stenosis degree of vertebral artery in the initial segment.
