摘 要:目的探究不孕症女性生殖道解脲脲原体(Uu)、沙眼衣原体(Ct)、人型支原体(Mh)、淋病奈瑟菌(NG)感染及与Th1/Th2相关性。方法选取2017年7月-2019年7月本院收治的95例不孕症患者作为观察组,纳入同期68例健康女性作为对照组。统计2组Uu、Ct、Mh、NG感染阳性率、Th1/Th2细胞因子水平,并对比Uu、Ct、Mh、NG感染与Th1/Th2细胞因子关系,受试者工作特征(ROC)曲线分析诊断价值,Pearson分析相关性。结果观察组继发性不孕患者Uu、Ct、Mh、NG感染率及血清IL-10、TNF-α、IFN-γ高于原发性不孕患者、对照组(P<0.05)。ROC结果显示,IL-10诊断不孕症AUC最大,为0.705。Uu、Ct、Mh、NG感染不孕症女性血清IL-10、TNF-α、IFN-γ高于无Uu、Ct、Mh、NG感染不孕症女性(P<0.05),且两者之间呈正相关(P<0.05).结论Uu、Ct、Mh、NG感染可通过影响Th1/Th2细胞因子水平,参与不孕症发生发展,及时检测治疗利于预防不孕症。Objective To investigate the infections of Ureaplasma urealyticum(UU),Chlamydia trachomatis(CT),Mycoplasma hominis(MH),Neisseria gonorrhoeae(NG)and their correlation with Th1/Th2 in the reproductive tract of infertile women.Methods A total of 95 infertility patients admitted to our hospital from July 2017 to July 2019 were selected as the observation group,and 68 healthy women were included as the control group during the same period.The positive rates of Uu,Ct,Mh,NG infection and Th1/Th2 cytokine levels in the two groups were counted,and the relationship between Uu,Ct,Mh,NG infection and Th1/Th2 cytokines was compared,the receiver operating characteristic(ROC)curve was used to analyze the diagnostic value,and Pearson was used to analyze the correlation.Results The rates of Uu,Ct,Mh,NG infections and serum IL-10,TNF-α,and IFN-γlevels in patients with secondary infertility in the observation group were higher than those in patients with primary infertility and the control group(P<0.05);ROC results showed that IL-10 had the largest AUC for infertility diagnosis at 0.705;the serum levels of IL-10,TNF-α,and IFN-γin women with Uu,Ct,Mh,and NG infections were higher than those in women without Uu,Ct,Mh,and NG infections,and there was a positive correlation between them(P<0.05).Conclusion Uu,Ct,Mh,and NG infections can participate in the development of infertility by affecting Th1/Th2 cytokine levels,and timely detection and treatment is conducive to the prevention of infertility.
