产妇产褥期感染的病原菌特点及耐药性分析

摘　　要：目的分析产妇产褥期感染的病原菌特点及对抗菌药物的耐药性。方法选取2017年1月-2019年9月在本院产科发生产褥期感染的22例产妇,采集留取相关血液、尿液、阴道分泌物或伤口分泌物等标本进行细菌学培养和体外药敏试验。结果22例发生产褥期感染产妇的感染部位最常见为宫内感染和阴道感染,检出24株病原菌,其中革兰阴性菌占66.67%(16/24),革兰阳性菌占29.17%(7/24),真菌感染占4.17%(1/24)。检出的革兰阴性菌对亚胺培南敏感,对氨苄青霉素耐药率最高,革兰阳性菌对氨苄青霉素耐药率最高,而对万古霉素和美罗培南敏感。结论产妇产褥期感染的病原菌以革兰阴性菌为主,对大多数抗生素耐药,临床应根据药敏结果选择合适的抗菌药物。Objective To analyze the characteristics of pathogenic bacteria in maternal puerperal infection and the resistance to antimicrobial agents.Methods Twenty-two women with sputum infections from the Department of Obstetrics and Gynecology in our hospital from January to September 2019 were selected to collect relevant blood,urine,vaginal secretions or wound secretions for bacteriology and in vitro susceptibility testing.Results The most common infections in 22 cases of intrauterine infection were intrauterine infection and vaginal infection.24 pathogens were detected,of which Gram-negative bacteria accounted for 66.67%(16/24)and Gram-positive bacteria accounted for 29.17%(7/24),fungal infections accounted for 4.17%(1/24).Gram-negative bacteria detected were sensitive to imipenem and had the highest resistance to ampicillin.Gram-positive bacteria had the highest resistance to ampicillin and sensitive to vancomycin and meropenem.Conclusion The pathogens of maternal puerperal infection are mainly Gram-negative bacteria,which are resistant to most antibiotics.Clinically,appropriate antibacterial drugs should be selected according to the results of drug susceptibility.
