摘 要:目的探讨高危型HPV感染对阴道菌群分布及宫颈细胞免疫的影响。方法根据是否感染高危型HPV,将186例患者分为高危型HPV阳性组(74例)和高危型HPV阴性组(112例),分析高危型HPV感染分型,阴道菌群分布及宫颈分泌物CD4^(+)T细胞、CD8^(+)T细胞分布差异性,高危型HPV感染与阴道菌群分布及宫颈细胞免疫的相关性。结果186例患者中,高危型HPV感染阳性率为39.78%,共检测出11种HPV亚型。高危型HPV阳性组解脲脲原体、衣原体、线索细胞检出率较高危型HPV阴性组显著增加(P<0.05),乳酸杆菌检出率较高危型HPV阴性组显著降低(P<0.05)。高危型HPV阳性组CD4^(+)T细胞百分比、CD4^(+)T细胞/CD8^(+)T细胞比值显著低于高危型HPV阴性组(P<0.05)。高危型HPV感染与解脲脲原体、衣原体、线索细胞呈正相关(P<0.05),而与乳酸杆菌、CD4^(+)T细胞百分比、CD4^(+)T细胞/CD8^(+)T细胞呈负相关(P<0.05)。结论高危型HPV感染可导致患者阴道菌群分布紊乱,降低宫颈细胞免疫功能,进而增加宫颈癌变风险。Objective To investigate the effect of high risk HPV infection on the distribution of vaginal flora and cervical cell immunity.Methods Depending on whether infection with high-risk HPV happens or not,186 patients were divided into high-risk HPV positive group(74 cases)and high-risk HPV negative group(112 cases).The types of high-risk HPV infection,the distribution of vaginal flora,the distribution of CD4^(+)T cells and CD8^(+)T cells in cervical secretion,and the correlation between high-risk HPV infection and the distribution of vaginal flora and cervical cell immunity were analyzed.Results In 186 patients,the positive rate of high-risk HPV infection was 39.78%,11 HPV subtypes were detected.The detection rate of Ureaplasma urealyticum,chlamydia and clue cells in the high-risk HPV positive group was significantly higher than that in the high-risk HPV negative group(P<0.05),and the detection rate of lactobacilli was significantly lower than that in the high-risk HPV negative group(P<0.05).The percentage of CD4^(+)T cells and the ratio of CD4^(+)T cells to CD8^(+)T cells in the high-risk HPV positive group were significantly lower than those in the high-risk HPV negative group.High risk HPV infection was positively correlated with Ureaplasma urealyticum,chlamydia and clue cell(P<0.05),but negatively correlated with Lactobacillus,CD4^(+)T cell percentage and CD4^(+)T cell/CD8^(+)T cell(P<0.05).Conclusion High risk HPV infection can lead to the disorder of vaginal flora distribution,decrease the immune function of cervical cells,and increase the risk of cervical cancer.
