摘 要:目的对义乌市2018年首例本地感染登革热病例开展调查及溯源分析,确定病毒的基因型及感染来源。方法对本地登革热病例进行流行病学调查,采集病例血清进行NS1抗原和病毒核酸检测,用RT-PCR法扩增全长E基因并测序,利用生物信息学软件进行多序列比对及构建进化树。结果该病例的NS1抗原和病毒核酸均为阳性,E基因序列对比及进化分析,该病毒株为登革病毒1型GⅠ亚型,与21株登革1型病毒的核苷酸和氨基酸相似性分别为90.3%~98.9%和95.8%~99.6%,与新加坡病毒株(登录号:MF033254)亲缘关系最近,核苷酸和氨基酸同源性分别为98.9%和99.6%。结论义乌市2018年首例本地感染登革热病例是由新加坡输入性传染源引起本地感染的可能性较大,需进一步加强东南亚输入性病例的监测和管理。Objective To understand the pathogen’s molecular characteristics of the first local Dengue fever case in Yiwu City in 2018 and analyze its molecular epidemiological characteristics. Methods The epidemiological survey was conducted and the serum sample was collected from the Dengue fever patient to detect dengue virus nucleic acids and NS 1 antigen. The envelope( E) gene of the virus was amplified and sequenced. The genotype of the isolate was analyzed with bioinformatics software.Results The serum samples of the patients was positive of virus nucleic acid and NS 1 antigen. The homologous and phylogenetic analysis showed that the isolate belonged to dengue virus type 1 and the sub genotype was GⅠ. The nucleotide and amino acid similarities were within 90. 3%-98. 9% and 95. 8%-99. 6% with 21 strains of Dengue type 1 virus,respectively. The nucleotide and amino acid homologies were 98. 9% and 99. 6% with Singapore strain( accessing number: MF033254).Conclusion The first locally infected case of Dengue fever in Yiwu City in 2018 was caused by the imported infectious source from Singapore,which is more likely to cause local infection. The surveillance and management of imported cases from Southeast Asia should be further strengthened.
