关节镜下内减压联合关节外囊肿切除术治疗半月板旁型囊肿的临床疗效

摘　　要：目的探讨关节镜下内减压联合关节外囊肿切除术治疗半月板旁型囊肿的临床疗效。方法对2013年9月至2018年12月在我院手术治疗的49例有症状的半月板旁型囊肿病人进行回顾性分析,其中行关节镜下内减压联合关节外囊肿切除术的27例(男19例,女8例)纳入观察组,行关节镜下内减压术的22例(男13例,女9例)纳入对照组。采用Lysholm评分和Glasgow评定法评估治疗效果。结果术后切口均一期愈合。观察组随访(26.7±13.4)个月,术前及末次随访的Lysholm评分分别为(61.4±7.6)分、(93.6±3.3)分,Glasgow评定结果:优25例,良2例。对照组随访(28.7±15.2)个月,术前及末次随访的Lysholm评分分别为(60.6±8.0)分、(88.6±4.3)分,Glasgow评定结果:优14例,良6例,一般2例。观察组的Lysholm评分和Glasgow评定结果均优于对照组,差异均有统计学意义(P均<0.05)。随访期间观察组囊肿无复发,对照组囊肿复发2例。结论关节镜下内减压联合关节外囊肿切除术是手术治疗膝关节半月板旁型囊肿的有效方法,相对关节镜下囊内减压术,具有囊肿不易复发,治疗彻底等优点,病人疼痛症状及膝关节功能有明显改善,临床疗效满意。Objective To explore the clinical effect of arthroscopic decompression combined with extra⁃articular cyst excision in the treatment of para⁃articular meniscus cyst.Methods From September 2013 to December 2018,49 patients with symptomatic para⁃articular meniscus cyst undergoing surgical treatment in our hospital were retrospectively analyzed.Among them,27 patients(19 males and 8 females)undergoing arthroscopic decompression combined with extra⁃articular cyst excision were included in the observation group,and 22 patients(13 males and 9 females)undergoing arthroscopic decompression were included in the control group.Lysholm score and Glasgow evaluation were used to evaluate the therapeutic effectiveness.Results All the incisions healed in stage I.In the observation group,the patients were followed up for(26.7±13.4)months;the Lysholm scores before and after operation were 61.4±7.6 and 93.6±3.3 respectively;for Glasgow evaluation results,there were 25 excellent cases and 2 good cases.In the control group,the patients were followed up for(28.7±15.2)months;the Lysholm scores before and after operation were 60.6±8.0 and 88.6±4.3 respectively;for Glasgow evaluation results,there were 14 excellent cases,6 good cases,and 2 general cases.The Lysholm score and Glasgow evaluation results in the observation group were better than those in the control group,and the differences were statistically significant(all P<0.05).During the follow⁃up period,there was no recurrence in the observation group,and 2 recurrence cases in control group.Conclusion Arthroscopic decompression combined with extra⁃articular cyst excision is an effective method for the treatment of para⁃articular meniscus cysts.Compared with arthroscopic decompression,arthroscopic decompression combined with extra⁃articular cyst excision has the following advantages:meniscus cyst is not easy to recur,and the treatment is complete.It can significantly improve the pain symptoms and knee joint function of patients,and the clinical effect is satisfact
