摘　　要：目的探讨思维导图结合集束化皮肤管理方案(SSKIN集束化管理方案)在预防老年髋关节置换病人医院获得性压疮(hospital⁃acquired pressure ulcer,HAPU)中的成效。方法采用便利抽样法,选择2017年6月至2019年5月武汉大学中南医院骨科收治的148例老年髋关节置换病人为研究对象。将2017年6月至2018年5月收治的72例病人纳入对照组,采用医院制定的集束化策略预防HAPU;2018年6月至2019年5月收治的76例病人纳入观察组,运用思维导图结合SSKIN集束化管理方案预防HAPU。观察评价两组病人的HAPU护理有效率、压疮预防知识掌握情况、Ⅰ期压疮发生率以及对护士HAPU护理满意度调查。结果观察组病人HAPU护理有效率(88.16%)和Ⅰ期压疮发生率(3.95%)均优于对照组(59.72%和19.44%),差异均有统计学意义(P均<0.05);观察组病人压疮预防知识掌握情况和对护士HAPU护理的满意度均优于对照组,差异均有统计学意义(P均<0.05)。结论思维导图结合SSKIN集束化管理方案的实施,保障了老年髋关节置换病人HAPU的预防护理质量,提高了病人的医疗护理安全与满意度,值得在临床应用及推广。Objective To explore the effect of the combination of mind mapping and SSKIN cluster skin management on preventing hospital⁃acquired pressure ulcers(HAPU)in elderly patients with hip replacement.Methods A total of 148 elderly patients with hip replacement in Zhongnan Hospital of Wuhan University from June 2017 to May 2019 were selected by convenient sampling method.Among them,72 patients from June 2017 to May 2018 were selected in the control group,and 76 patients from June 2018 to May 2019 were selected in the observation group.Patients in the control group were treated with the pressure injury cluster strategies developed by the hospital to prevent HAPU.The patients in the observation group were treated with the mind map combined with SSKIN cluster management scheme to prevent HAPU.The effective rate of HAPU nursing,the knowledge of pressure ulcers prevention,the incidence of stageⅠpressure ulcer and the satisfaction of nurses with HAPU nursing were observed and evaluated.Results The effective rate of HAPU nursing(88.16%)was higher and the incidence of stageⅠpressure ulcer(3.95%)was better in the observation group than those in the control group(59.72%and 19.44%),and the differences were statistically significant(both P<0.05).The mastery of pressure ulcer prevention knowledge and the satisfaction of nurses with HAPU nursing in the observation group were better than those in the control group,and the differences were statistically significant(both P<0.05).Conclusion The combination of mind mapping and SSKIN ensures the quality of pressure injury prevention and nursing care for elderly patients with hip replacement,improves the safety and satisfaction of medical care for patients,and is worthy of clinical promotion and application.
