摘 要:目的观察碳酸镧联合Supra-HFR血滤吸附治疗维持性血液透析(MHD)患者继发性甲状旁腺功能亢进(SHPT)的临床疗效。方法 80例MHD合并SHPT患者随机分为实验组和对照组:实验组给予口服碳酸镧+血液透析(2次/周)+Supra-HFR血滤吸附(1次/周),对照组给予口服碳酸镧+血液透析(3次/周)。比较两组治疗后4周血钙、磷、血清全段甲状旁腺激素(iPTH)水平。结果治疗后两组血钙水平均较同组治疗前升高(P<0.01),但组间差异无统计学意义(P>0.05);血磷水平明显下降,以实验组更为显著(P<0.05)。实验组治疗后血iPTH明显降低(P<0.01),而对照组则否。结论碳酸镧联合Supra-HFR血滤吸附可有效降低SHPT患者iPTH水平,改善钙磷代谢。Objective To observe the clinical efficacy of lanthanum carbonate and Supra-HFR hemofiltration adsorption in maintenance hemodialysis(MHD) patients with secondary hyperparathyroidism(SHPT). Methods Eighty MHD patients with SHPT were randomly treated with oral lanthanum carbonate + twice weekly hemodialysis + once weekly Supra-HFR hemofiltration absorption(experimental group) or oral lanthanum carbonate + thrice weekly hemodialysis(control group). Serum levels of calcium, phosphorus, and intact parathyroid hormone(iPTH) were compared between two groups 4 weeks post-treatment. Results Serum calcium content increased(P<0.01) post-treatment in both groups without a significant difference between two groups(P>0.05). Serum phosphorus level decreased(P<0.01) post-treatment in both groups, especially in experimental group(P<0.05). Serum iPTH level reduced post-treatment in experimental group(P<0.01) but not in control group. Conclusion Combined use of lanthanum carbonate and Supra-HFR hemofiltration absorption can effectively reduce serum iPTH level and improve calcium-phosphorus metabolism in MHD patients with SHPT.
