自制椭圆形纱球填塞治疗鼻腔后部出血的疗效观察

摘　　要：目的探讨自制椭圆形纱球填塞治疗对鼻腔后部出血患者的临床疗效。方法选取2018年12月至2019年3月于本院接受治疗的鼻腔后部出血患者50例,按照随机数字表法分为观察组与对照组,每组25例。对照组采用常规锥形纱球填塞治疗,观察组采用自制椭圆形纱球填塞治疗。比较两组临床疗效、一次止血治愈率、填塞物平均留置时间及视觉模拟评分法(visual analogue scale,VAS)评分。结果观察组治疗总有效率与1次止血治愈率(100.00%、76.00%)均高于对照组(80.00%、44.00%),差异有统计学意义(P<0.05);治疗后,观察组填塞物留置时间[(1.65±0.43)d]短于对照组[(3.56±1.02)d],差异有统计学意义(P<0.05);第1次填塞后1、6、12、24 h时,观察组VAS评分[(3.78±0.66)、(3.12±0.56)、(2.87±0.54)、(2.46±0.38)分]均低于对照组[(5.79±1.31)、(5.26±1.22)、(4.82±1.01)、(4.37±0.96)分],差异有统计学意义(P<0.05)。结论自制椭圆形纱球填塞治疗鼻腔后部出血的疗效确切,一次止血治愈率较高,填塞物留置时间较短,且不易引起不适感,具有较高的应用价值。Objective To explore the clinical effect of self-made oval-shaped gauze packing in patients with posterior nasal hemorrhage.Methods From December 2018 to March 2019,50 patients with posterior nasal hemorrhage were treated in our hospital,they were divided into observation group and control group according to random number table method,with 25 cases in each group.The control group was treated with conventional conical gauze packing,the observation group was treated with self-made oval-shaped gauze packing.Compared to clinical efficacy,cure rate of primary hemostasis,mean retention time of tamponade and visual analogue scale(VAS)score between two groups.Results The total effective rate of treatment in the observation group(100.00%,76.00%)was higher than that in the control group(80.00%,44.00%),the difference was statistically significant(P<0.05);after treatment,the retention time of packing in observation group[(1.65±0.43)d]shorter than(3.56±1.02)d in control group,the difference was statistically significant(P<0.05);1 h,6 h,12 h and 24 h after the first tamponade,VAS scores[(3.78±0.66),(3.12±0.56),(2.87±0.54),(2.46±0.38)scores]in the observation group were lower than(5.79±1.31),(5.26±1.22),(4.82±1.01),(4.37±0.96)scores in the control group,the difference was statistically significant(P<0.05).Conclusion The self-made oval-shaped gauze packing is effective in the treatment of posterior nasal bleeding.The cure rate of primary hemostasis is high,the days of indwelling stuffing are short,and it is not easy to cause discomfort.It has high application value.
