血清sFlt-1和PIGF检测早期诊断女性产前子痫的价值研究

摘　　要：目的分析检测血清血管内皮生长因子受体-1(sFlt-1)和胎盘生长因子(PIGF)对于早期诊断女性产前子痫前期的价值。方法选取本院2017年1月至2019年12月收治的94例产前子痫前期患者的血清标本作为观察组,将妊娠20~33+6周58例患者设为早发组,将妊娠34周~分娩前36例患者设为晚发组;选取同时段100名健康孕妇作为对照组。检测3组血清sFlt-1与PIGF水平及sFlt-1/PIGF比值,并比较3者对子痫前期的诊断价值。结果3组血清sFlt-1、PIGF及sFlt-1/PIGF比较差异有统计学意义(P<0.05),其中早发组血清sFlt-1与sFlt-1/PIGF均显著高于晚发组及对照组,且晚发组高于对照组(P<0.05);早发组血清PIGF显著低于晚发组及对照组,且晚发组低于对照组(P<0.05);轻度子痫前期患者的血清sFlt-1水平及sFlt-1/PIGF显著低于重度子痫前期组,其血清PIGF水平显著高于重度子痫前期组(P<0.05);sFlt-1、PIGF及sFlt-1/PIGF的ROC曲线下面积均>0.80,sFlt-1/PIGF诊断的灵敏度及特异度均高于sFlt-1、PIGF(P<0.05),总体诊断价值优于单项血清指标诊断。结论血清sFlt-1与PIGF水平在产前子痫患者体内变化明显,sFlt-1/PIGF可作为早期诊断产前子痫的重要指标,临床价值高。Objective To analyze the value of detecting serum vascular endothelial growth factor receptor-1(sFlt-1)and placental growth factor(PIGF)in the early diagnosis of prenatal female pre-eclampsia.Methods 94 patients with prenatal pre-eclampsia who were admitted to the hospital from January 2017 to December 2019 were enrolled as observation group,including 58 cases with gestational weeks of 20-33+6 weeks in early-onset group and 36 cases with gestational weeks of 34 weeks to pre-delivery in late-onset group.Another 100 healthy pregnant women during the same period were enrolled as control group.The levels of serum sFlt-1 and PIGF,andsFlt-1/PIGF among the three groups were detected.And the diagnostic value of the three indexes for pre-eclampsia was compared.Results There was significant difference in serum sFlt-1,PIGF and sFlt-1/PIGF among the three groups(P<0.05).The serum sFlt-1 and sFlt-1/PIGF in early-onset group were significantly higher than those in late-onset group and control group,which were higher in late-onset group than control group(P<0.05).The serum PIGF in early-onset group was significantly lower than that in late-onset group and control group,which was lower in late-onset group than control group(P<0.05).The level of serum sFlt-1 and sFlt-1/PIGF in mild pre-eclampsia group were significantly lower than those in severe pre-eclampsia group,while level of serum PIGF was significantly higher than that in severe pre-eclampsia group(P<0.05).The area under the ROC curve(AUC)values of sFlt-1,PIGF and sFlt-1/PIGF were all greater than 0.80.The diagnostic sensitivity and specificity of sFlt-1/PIGF were higher than those of sFlt-1 and PIGF(P<0.05),and the overall diagnostic value was better than that of single serum index.Conclusion The changes in levels of serum sFlt-1 and PIGF are significant in patients with prenatal pre-eclampsia.sFlt-1/PIGF can be applied as an important indexes for early diagnosis of prenatal pre-eclampsia,with high clinical value.
