摘　　要：目的探讨血清中IL-4、IL-6、TNF-α水平在儿童睡眠呼吸障碍中的价值。方法收集河南省人民医院和郑州大学第一附属医院2018年9月—2019年12月收治的68例儿童睡眠呼吸障碍资料,运用腺样体-鼻咽腔比比值,分为轻度阻塞组32例,中重度阻塞组36例,选取同期18例健康儿童为对照组。将轻度阻塞组、中重度阻塞组、对照组血清中IL-4、IL-6、TNF-α水平进行比较。结果轻度阻塞组血清中IL-4、IL-6、TNF-α水平显著高于对照组[3.71 vs 2.53;4.51 vs 3.44;4.86 vs 2.99,Z值分别为2.046;2.244;2.971,P<0.05],差异均有统计学意义;中重度阻塞组血清中IL-4、IL-6、TNF-α水平显著高于对照组[4.82 vs 2.53;5.53 vs 3.44;15.96 vs 2.99, Z值分别为3.028;3.936;8.332,P<0.05],差异均有统计学意义;中重度阻塞组血清中IL-6、TNF-α水平显著高于轻度阻塞组[5.53 vs 4.51;15.96 vs 4.86, Z值分别为2.027和4.681,P<0.05],差异均有统计学意义。结论血清中IL-4、IL-6、TNF-α水平升高有助于儿童睡眠呼吸障碍的诊断。Objective To investigate the value of serum IL-4, IL-6 and TNF-α levels in children with sleep-disordered breathing.Methods Data of 68 children with sleep-disordered breathing admitted from September 2018 to December 2019 were collected, and the adenoids-nasopharyngeal ratio was used to divide them into 32 cases with mild obstruction and 36 cases with moderate and severe obstruction. 18 healthy children in the same period were selected as the control group. Serum levels of IL-4, IL-6 and TNF-α were compared between the mild, moderate and severe occlusion groups and the control group.Results The serum levels of IL-4, IL-6 and TNF-α in the mildly clogged group were significantly higher than those in the control group[3.71 vs 2.53;4.51 vs 3.44;4.86 vs 2.99, with Z values of 2.046,2.244 and 2.971, P<0.05];The three indicators in the moderate and severe occlusion group were significantly higher than those in the control group[4.82 vs 2.53;5.53 vs 3.44;15.96 vs 2.99, with Z values of 3.028, 3.936 and 8.332, P<0.05].The serum levels of IL-6 and TNF-α in the moderate and severe occlusion group were significantly higher than those in the mild occlusion group[5.53 vs 4.51;15.96 vs 4.86, with Z values of 2.027 and 4.681, P<0.05], and the differences were statistically significant.Conclusion Elevated serum levels of IL-4, IL-6 and TNF-α contribute to the diagnosis of sleep-disordered breathing in children.
