冠心病采用平板运动心电图与多巴酚丁胺负荷超声诊断临床研究分析

摘　　要：目的分析平板运动心电图和多巴酚丁胺负荷超声诊断冠心病的临床效果。方法对2017年1月—2019年6月濮阳市油田总医院收治的50例疑似冠心病患者行冠状动脉造影、多巴酚丁胺负荷超声及平板运动心电图检查,以冠状动脉造影作为诊断冠心病的金标准,比较多巴酚丁胺超声心动图负荷试验及平板运动心电图负荷试验的准确性。结果冠状动脉造影检查显示50例疑似冠心病患者中36例为阳性,14例阴性,其中单支、双支冠状动脉病变分别为10例和21例,三支冠状动脉病变5例。多巴酚丁胺负荷超声检查的准确率为86.0%,显著高于平板运动心电图检查的68.0%,差异有统计学意义(P<0.05)。多巴酚丁胺负荷超声的诊断敏感性、特异性分别为86.1%和85.7%,与平板运动心电图的66.7%和71.4%比较,差异有统计学意义(P<0.05)。两种检查方法的阳性、阴性预测值比较,差异有统计学意义(P<0.05)。结论多巴酚丁胺负荷超声应用于冠心病诊断的准确率、敏感性和特异性均较高,能够为临床冠心病诊治提供重要依据,值得推广。Objective To analyze the clinical effect of treadmill exercise ecg and dobutamine stress ultrasound in the diagnosis of coronary heart disease.Methods From January 2017 to June 2019, totally 50 cases of our hospital patients with suspected coronary heart disease coronary angiography, ultrasound and dobutamine load of exercise ecg examination, coronary angiography as the gold standard in the diagnosis of coronary heart disease(CHD) and dobutamine stress echocardiography load test and exercise electrocardiogram test accuracy.Results Among the 50 patients with suspected CORONARY heart disease, 36 were positive and 14 were negative, including 10 patients with single coronary artery disease and 21 patients with double coronary artery disease, and 5 patients with triple coronary artery disease. The accuracy rate of dobutamine stress ultrasound examination was 86.0%, which was significantly higher than that of treadmill exercise ecg examination(68.0%). The diagnostic sensitivity and specificity of dobutamine stress ultrasound were 86.1% and 85.7% respectively, which were significantly different from those of 66.7% and 71.4% of treadmill electrocardiogram(P<0.05). The positive and negative predictive values of the two methods were significantly different(P<0.05).Conclusion Dobutamine stress ultrasound has high accuracy, sensitivity and specificity in the diagnosis of coronary heart disease, which can provide an important basis for the diagnosis and treatment of clinical coronary heart disease, and is worthy of promotion.
