摘 要:目的分析尿微量蛋白测定在小儿过敏性紫癜早期肾损害中的意义,为过敏性紫癜早期肾损害患儿找到一种有效的诊断方法。方法选择我院自2018年1月至2020年7月收治的220例过敏性紫癜患儿作为研究对象,按其是否存在尿常规异常分为单纯过敏性紫癜组(对照组,120例)和过敏性紫癜合并尿常规异常组(研究组,100例),两组患儿均行尿微量蛋白测定,对比两组患儿的尿微量白蛋白含量(UAlb)、β2-微球蛋白含量(β2-MG)、24h尿总蛋白定量值(UPRO)及UAlb、β2-MG、UPRO三项指标的阳性检出率。结果研究组患儿UAlb、β2-MG、UPRO三项指标水平均显著高于对照组,两组对比具有统计学意义(P<0.05);UAlb、β2-MG两项指标的阳性检出率对比无显著差异(P>0.05),UAlb、β2-MG两项指标的阳性检出率均显著高于UPRO,组间对比具有统计学意义(P<0.05);尿微量白蛋白阳性率显著高于尿常规、Addis计数阳性率,组间对比具有统计学意义(P<0.05)。结论尿微量蛋白测定在小儿过敏性紫癜早期肾损害中具有重要的诊断意义,能够提高肾损伤早期诊断阳性率,从而能指导临床尽早采取有效方案对过敏性紫癜早期肾损害患儿进行治疗。Objective To analyze the significance of urinary microprotein in early renal damage in children with Henoch Schonlein purpura,and to find an effective diagnostic method for early renal damage in children with Henoch Schonlein purpura.Methods 220 cases of children with Henoch Schonlein purpura in our hospital from January 2018 to July 2020 were selected as the research objects,and they were divided into simple Henoch Schonlein purpura group(control group,120 cases)and Henoch Schonlein Purpura Combined with abnormal urine routine(study group,100 cases)according to whether there were abnormal urine routine The positive detection rates of UAlb,β2-mg and Upro were detected.Results The levels of UAlb,β2-mg and Upro in the study group were significantly higher than those in the control group,with statistical significance(P<0.05);there was no significant difference in the positive detection rate of UAlb andβ2-mg(P>0.05),and the positive detection rates of UAlb andβ2-mg were significantly higher than those of Upro,with statistical significance between groups(P<0.05);urinary microalbumin was positive The positive rate was significantly higher than that of routine urine test and Addis count(P<0.05).Conclusion Urine Microprotein Detection has important diagnostic significance in early renal damage of children with Henoch Schonlein purpura,which can improve the positive rate of early diagnosis of renal injury,and guide clinical to take effective treatment for early renal damage in children with Henoch Schonlein purpura.
