摘 要:目的探讨血浆微小RNA-127(miR-127)、白细胞介素-32(IL-32)水平对急性肺损伤(ALI)患者预后的预测价值。方法选取233例ALI患者为观察组,根据患者死亡或痊愈出院分为死亡组87例和存活组146例,分别采用RT-PCR法、酶联免疫吸附法测定ALI患者血浆miR-127、IL-32水平,收集患者临床资料,多因素Logistic回归分析ALI患者死亡危险因素;绘制受试者工作特征(ROC)曲线,分析血浆miR-127、IL-32水平对ALI患者死亡的预测价值。结果观察组血浆miR-127、IL-32水平高于对照组(P均<0.05);死亡组平均动脉压(MAP)低于存活组,呼气末正压(PEEP)和血浆miR-127、IL-32水平高于存活组(P均<0.05);多因素Logistic回归分析显示,miR-127(OR=2.673,95%CI:1.834~7.749)和IL-32(OR=1.269,95%CI:1.025~1.570)为ALI患者死亡的独立危险因素(P均<0.05);ROC曲线分析显示,血浆miR-127+IL-32水平(AUC=0.913,95%CI:0.869~0.946)预测ALI患者死亡的灵敏度、特异度、准确度高于miR-127(AUC=0.829,95%CI:0.775~0.875)、IL-32(AUC=0.814,95%CI:0.758~0.862)单独预测。结论ALI患者血浆miR-127、IL-32水平升高,联合测定血浆miR-127、IL-32水平能提高对ALI患者预后的预测价值。Objective To investigate the predictive value of plasma microRNA-127(miR-127)and interleukin-32(IL-32)levels on the prognosis of patients with acute lung injury(ALI).Methods A total of 233 ALI patients were selected as the observation group,and then they were divided into the death group(87 cases)and survival group(146 cases)according to the patients'death or recovery of discharge status.The plasma levels of miR-127 and IL-32 were measured by RT-PCR and ELISA,and the clinical data were collected.Multivariate Logistic regression analysis was used to analyze the risk factors for death in patients with ALI,and the receiver operating characteristic(ROC)curve was drawn.The predictive value of plasma miR-127 and IL-32 levels on the death of patients with ALI was analyzed.Results The levels of plasma miR-127 and IL-32 in the observation group were significantly higher than those in the control group(both P<0.05);the mean arterial pressure(MAP)in the death group was significantly lower,and the positive end-expiratory pressure(PEEP)and plasma miR-127,IL-32 levels were significantly higher than those of the survival group(all P<0.05);multivariate Logistic regression analysis showed that miR-127(OR=2.673,95%CI:1.834-7.749)and IL-32(OR=1.269,95%CI:1.025-1.570)were independent risk factors for death of ALI patients(both P<0.05);ROC curve showed that the sensitivity,specificity,and accuracy of plasma miR-127+IL-32 level(AUC=0.913,95%CI:0.869-0.946)were higher than those of miR-127(AUC=0.829,95%CI:0.775-0.875)and IL-32(AUC=0.814,95%CI:0.758-0.862)alone in the prediction of death of ALI patients.Conclusion Plasma miR-127 and IL-32 levels in ALI patients increased,and the combined detection of plasma miR-127 and IL-32 levels can improve the prognostic value for ALI patients.
