成人肺先天性囊性腺瘤样畸形的CT表现及鉴别诊断

摘　　要：目的探讨CT对成人肺先天性囊性腺瘤样畸形(CCAM)的诊断价值。方法回顾性分析经手术病理证实的6例成人肺CCAM的胸部CT表现,其中4例行CT平扫及增强扫描,另2例仅行CT平扫。结果右肺受累4例,左肺受累2例。成人肺CCAM的CT表现为囊性或囊实性病变,包括较大薄壁囊状透亮区、蜂窝状多发薄壁空腔及软组织肿块影伴周围多发小囊。增强扫描病灶实性部分及囊壁呈较均匀强化,部分囊腔内见气-液平面。CCAM病灶均有不同程度的占位效应。结论CT可显示成人肺CCAM的病变大小、位置、性质及周围肺组织改变,是诊断成人肺CCAM的可靠方法。Objctive To explore the diagnostic value of CT in congenital cystic adenomatoid malformation(CCAM)of the lung in adults.Methods Retrospective analysis of CT features in 6 pathologically proven cases with CCAM of the lung in adults.Plain and contrast-enhanced CT scanning were performed in 4 cases while only plain chest CT scanning was performed in other 2 cases.Results Right lung involved in 4 cases and left lung involved in 2 ases.The CT features of CCAM of the lung in adults were cystic or cystic solid lesions,including large thin-walled air cyst,honeycomb-like multiple small air cysts,and solid mass surrounded by multiple small cysts.The solid portion and cyst wall showed homogeneous enhancement on contrast-enhanced CT images.Some cystic lesions showed air-fluid levels.The CT imaging showed all the lesions of CCAM with certain space-occupying efect.Concusion CT can clearly show the size。location and characteristic of CCAM and surrounding lung tissue,and it is a reliable method for diagnosing CCAM of the lung in adults.
