摘　　要：目的探讨MRI和超声检查对输卵管积液的诊断价值对比分析。方法回顾性分析经病理或临床确诊为输卵管积液的36例患者46条积液输卵管的MRI及超声检查资料。对比分析MRI和超声成像在输卵管积液影像学征象的表现差异,评价影像学检查对输卵管积液诊断效能。结果36例46个病灶中,单纯性输卵管积水41条,积脓输卵管5条,双侧发病10例,单侧26例。2种检查方法对输卵管积液的形态描述分别为:腊肠样改变(20-14),管样合并囊状改变(3-4),囊状改变(23-24),二者对形态的诊断无明显统计意义(P>0.05);输卵管积脓扩散加权成像(DWI)表现为高信号,表观扩散系数(ADC)值平均约1.27×10^(-3)mm^(2)/s,输卵管积液DWI表现为低信号,ADC值平均约2.42×10^(-3)mm^(2)/s。MRI及超声检查对输卵管积液诊断的准确度分别为86.96%、69.57%,差异有统计学意义(χ^(2)=4.08,P<0.05)。MRI和超声检查对输卵管积液形态、液-液分层等的诊断效能均较高,但超声检查难以准确区分输卵管积水/积脓的性质。结论输卵管积液的MRI及超声表现具有一定特征性,两者均可反映输卵管积液的形态、盆腔情况等,MRI可准确反映输卵管积液的性质及与邻近组织脏器的关系,对该病的诊断和鉴别诊断具有重要价值。Oljetive To study the diagnostic value of MRI and utrasonograply in hyroslpinxMthols The MRI and utrasoography data of 36 patients with 46 hydrosalpinx confirmed by pathology or clinic were analyzed retrospectively.To compare the differences of imaging features and evaluate the diagnostie fficacy between MRI and ultrasonography in hydrosalpinx.Results Of the 46 lesions in 36 cases,41 were simple hydrosalpinx,5 were empyema oviduct,10 were bilateral and 26 were unilateral.The two methods described the morphology of hydrosalpinx as fllws:sausage like changes(20:14),tubular and cystie-shaped(3:4).pouch-changed(23:24),no statistical diferences between them in shape diagnosis(P>0.05).The dffusion weighted imaging(DWI)of pyosalpinx showed high signal and the mean apparent diffusion cefficient(ADC)value was about 1.27×10^(-3) mm^(2)/s.The DWI of hydrosalpinx was low signal and the mean ADC value was about 2.42×10^(-2) mm^(2)/s.The accuracy of MRI and ultrasonography in diagnosis of hydrosalpinx was 86.96%,69.57% respectively,and the differences in diagnostic accuracy was statistically(Х^(2)=4.08,P<0.05).The diagnostic eficacy of MRI and ultrasonography in morphology and stratification of hydrosalpinx was high,but it was dfficult to distinguish the nature of bydrosalpinx or pyosalpinx accurately by ultrasonography.Concdusion The MRI and ultasound manifestations of hydrosalpinx have some characteristics.Both of therm can rflet the morphology of bhydrosalpinx and the condition of pelvic cavity.However。MRI can accurately rflect the nature of hydrosalpinx and the relationship between bhydrosalpinx and adjacent tssues and organs.It is of great value in diagnosis and dfferential diagnosis of hydrosalpinx.
